What is Pulse Pressure?

Pulse pressure is the numerical difference between the systolic blood pressure (the upper number in a blood pressure reading) and the diastolic blood pressure (the lower number). It is measured in millimeters of mercury (mmHg).

Example: A blood pressure of 120/80 mmHg gives a pulse pressure of 40 mmHg (120 minus 80 = 40).

A normal pulse pressure is generally considered to be between 30 and 50 mmHg. Values outside this range may indicate underlying cardiovascular conditions.

Clinical Significance

Pulse pressure is a key indicator of arterial stiffness. As people age, arterial walls lose their elasticity, causing pulse pressure to rise. An elevated pulse pressure is a recognized risk factor for cardiovascular events such as heart attack, stroke, and heart failure.

Causes of Elevated Pulse Pressure

  • Arterial stiffness: Reduced elasticity of arterial walls with aging is the most common cause.
  • Atherosclerosis: Plaque buildup in vessel walls increases vascular rigidity.
  • Aortic regurgitation: A leaky aortic valve elevates systolic pressure.
  • Hyperthyroidism: Increases heart rate and alters blood pressure dynamics.
  • Severe anemia: Compensatory increases in cardiac output can raise pulse pressure.
  • Chronic kidney disease: Affects fluid balance and vascular resistance.

Causes of Reduced Pulse Pressure

  • Heart failure: Reduced cardiac output lowers systolic pressure.
  • Valvular stenosis: Narrowed heart valves reduce stroke volume.
  • Shock: Both cardiogenic and hypovolemic shock can significantly lower pulse pressure.
  • Cardiac tamponade: Fluid accumulation around the heart impairs cardiac function.

Diagnosis and Measurement

Pulse pressure is not measured directly but is calculated from a standard blood pressure measurement by subtracting the diastolic value from the systolic value. For a reliable assessment, physicians recommend multiple measurements at different times of day, and in some cases a 24-hour ambulatory blood pressure monitoring is advised.

Treatment and Management

Treatment of abnormal pulse pressure always targets the underlying cause:

  • Lifestyle changes: Regular physical activity, a heart-healthy diet, weight management, and smoking cessation can improve arterial elasticity.
  • Medications: Antihypertensive agents such as ACE inhibitors, calcium channel blockers, or beta-blockers may be prescribed depending on the cause.
  • Treatment of underlying conditions: Conditions such as aortic regurgitation, hyperthyroidism, or renal failure require targeted therapy.
