The supratrochlear foramen is a rounded or oval opening in the distal humerus, located above the trochlea humeri. It results from the absence of the bony septum that normally separates the coronoid fossa (anterior) from the olecranon fossa (posterior). When this thin bony partition is completely absent, a through-hole is formed. This feature is considered an anatomical normal variant and is not a pathological finding.

Anatomical Background

The distal humerus features two bony depressions on opposite surfaces:

  • Coronoid fossa: Located on the anterior surface; accommodates the coronoid process of the ulna during elbow flexion.
  • Olecranon fossa: Located on the posterior surface; accommodates the olecranon of the ulna during elbow extension.

Under normal circumstances, these two fossae are separated by a thin bony septum. When this septum is completely absent, the supratrochlear foramen forms as a continuous opening through the bone.

Prevalence and Distribution

The supratrochlear foramen is a well-documented anatomical variant whose frequency varies across populations. It is more commonly observed in Asian and African populations and is less frequent in European populations. The foramen may be unilateral or bilateral and has been reported more frequently in females than in males.

Clinical Relevance

In most cases, the supratrochlear foramen is asymptomatic and clinically insignificant. However, it carries importance in the following contexts:

  • Radiological imaging: On conventional X-rays of the elbow, the foramen may appear as a radiolucent zone and can be misinterpreted as a pathological lesion such as a bone cyst, tumor, or fracture.
  • Forensic and physical anthropology: This feature is used in skeletal analysis to help determine population affinity and biological sex.
  • Surgical planning: Awareness of this variant is important during elbow or distal humerus surgery to avoid misinterpretation of intraoperative findings.

Diagnosis

The supratrochlear foramen is typically identified incidentally during imaging of the elbow joint:

  • X-ray: Reveals an oval or round radiolucent area in the distal humerus above the trochlea.
  • Computed tomography (CT): Confirms the complete absence of the interseptal bone and rules out pathological causes.
  • MRI: May be used in differential diagnosis to definitively exclude other lesions.

Differential Diagnosis

Because the supratrochlear foramen can appear unusual on radiographs, careful differential diagnosis is essential. It must be distinguished from:

  • Bone cysts
  • Enchondromas
  • Fractures of the distal humerus
  • Osteolytic bone metastases

Recognizing this anatomical variant is key to avoiding unnecessary further investigations or inappropriate treatment decisions.
