Things worth knowing about "T-Score in osteoporosis"

The T-score is a key measurement used to assess bone density and diagnose osteoporosis. It shows how much a person's bone density differs from the average of healthy young adults.

What is the T-Score in Osteoporosis?

The T-score is a statistical value obtained during a bone density measurement (osteodensitometry). It compares an individual's bone mineral density (BMD) to the average BMD of a healthy young adult between 20 and 30 years of age. The result is expressed in standard deviations (SD) and is the primary diagnostic criterion for osteoporosis and osteopenia (reduced bone density as a precursor to osteoporosis).

How is the T-Score Measured?

The T-score is calculated based on a DXA scan (Dual-energy X-ray Absorptiometry), which is the gold-standard method for measuring bone mineral density. The scan typically evaluates the following areas:

  • Lumbar spine
  • Femoral neck (hip)
  • Total hip

A DXA scan is painless, uses very low radiation, and takes only a few minutes. The resulting T-score indicates how many standard deviations above or below the young adult reference value the individual's bone density falls.

How to Interpret the T-Score

The World Health Organization (WHO) has established the following classification for postmenopausal women and older men:

  • T-score ≥ -1.0: Normal bone density – no action required
  • T-score between -1.0 and -2.5: Osteopenia – reduced bone density, increased risk of future osteoporosis
  • T-score ≤ -2.5: Osteoporosis – significantly reduced bone density, elevated fracture risk
  • T-score ≤ -2.5 with at least one fracture: Severe (established) osteoporosis

The lower the T-score, the lower the bone density and the greater the risk of fractures, particularly of the spine, hip, and wrist.

T-Score vs. Z-Score

In addition to the T-score, there is the Z-score, which compares bone density to the age- and sex-matched average. The Z-score is used primarily for younger individuals, children, adolescents, and premenopausal women, as the T-score is less meaningful in these groups.

Who Should Have a Bone Density Test?

A bone density test and T-score evaluation is recommended for:

  • Women after menopause, especially from age 65 onward
  • Men aged 70 and older
  • Individuals with known risk factors such as long-term corticosteroid use, physical inactivity, or a family history of osteoporosis
  • Those who have experienced a low-trauma fracture (broken bone from minimal force)
  • Patients with conditions affecting bone metabolism (e.g., rheumatoid arthritis, hyperthyroidism)

Treatment for a Low T-Score

Depending on the severity of the T-score result, different measures may be recommended:

General Measures

  • Adequate intake of calcium (approximately 1000–1200 mg per day) and vitamin D
  • Regular physical activity, especially strength and balance training
  • Avoiding smoking and excessive alcohol consumption
  • Fall prevention strategies in daily life

Medical Treatment

  • Bisphosphonates (e.g., alendronate, zoledronic acid): inhibit bone resorption
  • Denosumab: a monoclonal antibody that reduces bone breakdown
  • Teriparatide: stimulates new bone formation (used in severe osteoporosis)
  • Romosozumab: combines bone-building and bone-resorption-inhibiting effects
