What is TBE?

Tick-borne encephalitis (TBE) is a viral infection of the central nervous system transmitted primarily through the bite of infected ticks (Ixodes ricinus). The causative agent is the TBE virus, a member of the Flavivirus family. The disease is most prevalent in endemic regions of Central and Eastern Europe, Scandinavia, and parts of Asia.

Course of the Disease

TBE typically progresses in two phases:

  • First phase: Flu-like symptoms including fever, fatigue, headache, and muscle aches lasting approximately 2–7 days.
  • Second phase: In around 10–30% of cases, after a symptom-free interval, neurological involvement occurs. This may manifest as meningitis, encephalitis, or myelitis, with symptoms such as high fever, severe headache, neck stiffness, impaired consciousness, or paralysis.

Treatment of TBE

Currently, there is no specific antiviral treatment available for TBE. Management is therefore entirely symptomatic and supportive, aimed at relieving symptoms and preventing complications.

General Measures

  • Rest and reduced physical activity during the acute phase.
  • Adequate fluid intake to maintain circulation and hydration.
  • Close medical monitoring, especially in patients with neurological symptoms.

Symptomatic Drug Therapy

  • Analgesics and antipyretics (e.g., paracetamol or ibuprofen) to manage fever and pain.
  • Antiemetics to control nausea and vomiting.
  • Corticosteroids may be considered in severe cases with significant brain inflammation, although their benefit has not been conclusively established.
  • Anticonvulsants are used in patients experiencing seizures.

Inpatient Care

Patients with severe neurological involvement are treated in hospital, often in an intensive care unit. Measures include:

  • Continuous monitoring of vital signs and neurological status.
  • Mechanical ventilation in cases of respiratory failure.
  • Physiotherapy and rehabilitation for persisting neurological deficits.

Rehabilitation

After severe disease courses, neurological rehabilitation may be required. Memory difficulties, concentration problems, paralysis, or balance disorders can persist for months and require targeted support through physiotherapy, occupational therapy, and speech therapy.

Prevention: The Most Important Measure

Since no causal treatment exists, prevention is paramount:

  • TBE vaccination: The vaccine is the most effective preventive measure. It is recommended in endemic regions and consists of three doses with regular booster injections.
  • Tick protection: Wearing long clothing, using insect repellents, and performing thorough body checks after outdoor activities significantly reduce the risk of tick bites.
  • Prompt tick removal: The TBE virus can be transmitted within minutes of a bite, so ticks should be removed as quickly as possible.
