Thymosin beta-4 (also referred to as Tβ4 or its synthetic analogue TB-500) is a small, biologically active peptide consisting of 43 amino acids, originally isolated from the thymus gland. It is found in virtually all human tissues and body fluids and is naturally produced by the body. Thymosin beta-4 is considered one of the most important tissue-regenerative peptides and is the subject of extensive biomedical research worldwide.

Clinical studies have indicated that thymosin beta-4, administered topically or systemically, can accelerate the healing of chronic wounds, burns, and corneal defects. Positive results have been particularly reported in patients with diabetic foot ulcers.
In preclinical models, thymosin beta-4 improved the survival of cardiomyocytes following myocardial infarction and helped preserve cardiac function. Phase I and Phase II clinical trials have already been conducted in this area.
Thymosin beta-4 is being investigated as an active ingredient in eye drops for the treatment of corneal erosions and dry eye syndrome. A related investigational product (RGN-259) has undergone clinical evaluation.
Experimental studies suggest that thymosin beta-4 may support remyelination of nerve fibers in diseases such as multiple sclerosis, although further clinical evidence is needed.
The synthetic analogue TB-500 has been misused in competitive sports due to its attributed regenerative and performance-enhancing properties. The World Anti-Doping Agency (WADA) has included thymosin beta-4 on its list of prohibited substances. Its use outside of controlled clinical trials is therefore banned in sport and carries health risks due to uncontrolled dosing and insufficiently studied long-term effects.
In clinical studies, thymosin beta-4 has generally demonstrated a favorable safety profile. Reported side effects include mild local reactions at the injection site, transient fatigue, and occasional headaches. Long-term human safety data remain limited. Its use should be restricted to clinical trial settings or supervised medical practice.
Thymosin beta-4 is the subject of numerous ongoing clinical trials. RegeneRx Biopharmaceuticals has conducted multiple Phase I and Phase II studies. Research continues to focus on optimizing delivery methods, dosing strategies, and identifying additional therapeutic applications. Despite promising preclinical results, broad clinical approval has not yet been granted.
