What is Torasemide?

Torasemide (also spelled torsemide) is a medication belonging to the class of loop diuretics. It acts on the kidneys to promote the increased excretion of water, sodium, and chloride in the urine. Torasemide is widely used in the management of edema (fluid retention) and arterial hypertension (high blood pressure).

Indications

Torasemide is prescribed for the following conditions:

  • Heart failure accompanied by fluid accumulation in the body tissues
  • Liver disease such as cirrhosis with ascites (fluid in the abdominal cavity)
  • Kidney disease with edematous symptoms
  • Arterial hypertension (high blood pressure)

Mechanism of Action

Torasemide inhibits the Na-K-2Cl cotransporter (NKCC2) located in the thick ascending limb of the loop of Henle in the kidney. By blocking this transporter, the reabsorption of sodium, potassium, and chloride back into the bloodstream is prevented. These ions remain in the tubular fluid and osmotically draw water along with them, resulting in increased urine output (diuresis). Torasemide also has mild vasodilatory properties, which contribute to its blood pressure-lowering effect.

Dosage

Dosage is determined individually by the treating physician based on the severity of the condition and kidney function. Common dosage ranges include:

  • Edema due to heart failure: 5-20 mg daily, up to 200 mg in severe cases
  • Edema due to liver or kidney disease: 5-20 mg daily
  • Arterial hypertension: 2.5-5 mg daily

Torasemide is most commonly administered as an oral tablet. An intravenous form is also available for clinical use.

Side Effects

Like all medications, torasemide can cause side effects. The most common include:

  • Electrolyte imbalances: Hypokalemia (low potassium), hyponatremia (low sodium), and hypomagnesemia
  • Dehydration due to excessive fluid loss
  • Dizziness, headache, and muscle cramps
  • Elevated blood glucose and uric acid levels
  • Rarely: hearing impairment, especially at very high doses

Regular blood tests and kidney function monitoring are recommended during treatment with torasemide.

Drug Interactions

Torasemide may interact with other medications. Important interactions include:

  • ACE inhibitors and angiotensin II receptor blockers: enhanced blood pressure-lowering effect
  • Cardiac glycosides (e.g., digoxin): increased risk of arrhythmias in the presence of hypokalemia
  • Nonsteroidal anti-inflammatory drugs (NSAIDs): reduced diuretic effect and increased risk of kidney damage
  • Aminoglycoside antibiotics: increased risk of ototoxicity and nephrotoxicity
  • Lithium: reduced lithium clearance, increasing the risk of lithium toxicity

Contraindications

Torasemide should not be used in patients with:

  • Known hypersensitivity to torasemide or sulfonamide derivatives
  • Anuria (absence of urine production) or prerenal kidney failure
  • Severe hypokalemia or hyponatremia
  • Pregnancy and breastfeeding, unless strictly indicated after risk-benefit assessment
