Transient elastography (widely known by the brand name FibroScan®) is a modern, non-invasive diagnostic method that measures the stiffness (elasticity) of liver tissue. Since increasing liver stiffness is typically associated with progressive liver fibrosis – an abnormal accumulation of scar tissue in the liver – this technique provides valuable information about the health of the liver. It is used worldwide as a standard method in hepatology and, in many cases, replaces the more burdensome liver biopsy.
During transient elastography, a specialized ultrasound probe is placed on the skin over the right side of the ribcage. The device transmits a mild, controlled mechanical vibration along with an accompanying ultrasound pulse into the liver tissue. Based on the speed at which this shear wave travels through the tissue, the system calculates tissue stiffness in kilopascals (kPa).
Modern devices can also measure the CAP value (Controlled Attenuation Parameter), which quantifies the fat content of the liver (steatosis).

The procedure is painless and typically takes only a few minutes. The patient lies relaxed on their back with the right arm raised behind the head. A gel is applied to the skin to ensure good contact with the probe. The device automatically performs multiple measurements (typically 10 valid readings), from which a median value is calculated. Fasting prior to the examination may be recommended depending on the clinical protocol, as recent meals can temporarily increase stiffness values.
