What Is a Weight Loss Injection?

The term weight loss injection refers to a class of prescription medications used to treat obesity and related metabolic conditions. The most well-known active ingredients are semaglutide (e.g., Ozempic, Wegovy) and tirzepatide (e.g., Mounjaro). These medications are injected under the skin once a week and mimic naturally occurring hormones that regulate blood sugar levels and promote feelings of fullness.

While weight loss injections have demonstrated clinically proven benefits for weight reduction, they are not without risks. Medical supervision throughout the entire course of treatment is essential.

Common Risks and Side Effects

Gastrointestinal Complaints

The most frequently reported side effects involve the gastrointestinal tract and include:

  • Nausea and vomiting
  • Diarrhea or constipation
  • Abdominal pain and bloating
  • Heartburn and belching

These symptoms are most common at the start of treatment and tend to diminish over time. A gradual dose escalation can help minimize these effects.

Hypoglycemia (Low Blood Sugar)

Particularly when combined with other blood-sugar-lowering medications such as insulin or sulfonylureas, there is a risk of hypoglycemia – a dangerous drop in blood glucose levels. Symptoms include trembling, sweating, dizziness, and loss of consciousness.

Pancreatitis

In rare cases, pancreatitis (inflammation of the pancreas) has been reported. Severe, persistent abdominal pain – especially if it radiates to the back – requires immediate medical evaluation. Patients with a history of pancreatic disease should not use these medications.

Thyroid Tumors (Theoretical Risk)

Animal studies have indicated that GLP-1 receptor agonists may promote the growth of thyroid tumors. Whether this risk applies to humans has not yet been conclusively established. Individuals with a personal or family history of medullary thyroid carcinoma or MEN-2 syndrome must not use these medications.

Gallbladder Problems

Rapid weight loss can lead to the formation of gallstones. Pain in the upper right abdomen should prompt immediate medical consultation.

Increased Heart Rate

Some users experience a slightly elevated resting heart rate. Patients with pre-existing cardiac conditions should only use these medications under close cardiological supervision.

Psychiatric Side Effects

Isolated cases of depressive episodes, mood changes, and – very rarely – suicidal thoughts have been reported. Any psychological changes during treatment should be discussed with a doctor immediately. Current reviews by regulatory authorities (EMA, FDA) have not yet established a definitive causal link, but ongoing monitoring is in place.

Serious and Rare Risks

  • Kidney problems: Severe nausea and vomiting can lead to dehydration and, consequently, kidney damage.
  • Allergic reactions: Rash, itching, or swelling of the face and throat require immediate medical attention.
  • Gastroparesis (stomach paralysis): Delayed gastric emptying can occur as a rare complication and may complicate surgical procedures due to incomplete stomach emptying.

Who Should Not Use Weight Loss Injections?

Weight loss injections are not suitable for:

  • Pregnant or breastfeeding women
  • Individuals with a personal or family history of medullary thyroid carcinoma or MEN-2 syndrome
  • Individuals with active or chronic pancreatitis
  • Minors (except in specifically approved cases)
  • Individuals without a medical prescription and professional supervision

Important Notes on Safe Use

Weight loss injections are prescription-only medications and must only be used under medical supervision. Self-sourcing through unauthorized online channels is dangerous and illegal in many countries. Regular check-ups – including blood counts, kidney function tests, and blood glucose monitoring – are required throughout the entire treatment period. Treatment should always be accompanied by dietary changes and increased physical activity.
