What is Xanax?

Xanax is the brand name for the active ingredient alprazolam, a medication belonging to the benzodiazepine class. It is available by prescription only and is used in psychiatry and general medicine. Xanax produces sedative, anxiolytic (anti-anxiety), muscle-relaxant, and sleep-inducing effects.

Indications

Xanax is primarily prescribed for the following conditions:

  • Anxiety disorders (generalized anxiety disorder, social phobia)
  • Panic disorder – with or without agoraphobia
  • Short-term relief of severe anxiety symptoms
  • In some cases, as adjunct therapy in depression with an anxiety component

Mechanism of Action

Alprazolam, the active substance in Xanax, enhances the effect of the inhibitory neurotransmitter GABA (gamma-aminobutyric acid) in the brain. It binds to specific GABA-A receptors, increasing the frequency of chloride ion channel opening. This results in a dampening of central nervous system activity, producing feelings of calm, reduced anxiety, and relaxation.

Dosage

Dosage is determined individually by a physician. Typical adult dosages include:

  • Anxiety disorders: 0.25 mg to 0.5 mg, three times daily
  • Panic disorder: up to 10 mg per day in severe cases, under close medical supervision
  • Treatment duration should be kept as short as possible – generally no longer than 8 to 12 weeks

Xanax is not suitable for long-term use due to the significantly increased risk of dependence with prolonged administration.

Side Effects

Like all benzodiazepines, Xanax can cause a range of side effects:

  • Common: Drowsiness, dizziness, difficulty concentrating, impaired coordination
  • Occasional: Memory impairment, mood changes, confusion
  • Rare: Paradoxical reactions such as agitation, aggression, or euphoria
  • With long-term use: Physical and psychological dependence, tolerance development, withdrawal symptoms upon discontinuation

Dependence Potential and Risks

Xanax carries a high risk of addiction. Physical dependence can develop within just a few weeks of regular use. Discontinuation should therefore never be abrupt but always done gradually under medical supervision (tapering). Withdrawal symptoms may include anxiety, insomnia, tremors, and in severe cases, seizures.

Xanax must not be combined with alcohol or other central nervous system depressants, as this can lead to dangerous interactions. Use during pregnancy and breastfeeding is contraindicated.

Treatment Context

Xanax is intended as a short-term crisis intervention tool, not a long-term solution. Specialists typically recommend the following alongside or instead of Xanax:

  • Psychotherapy (particularly cognitive behavioral therapy)
  • Long-term pharmacotherapy with SSRIs or SNRIs for anxiety disorders
  • Relaxation techniques such as progressive muscle relaxation or breathing exercises
