What Is the Yolk Sac?

The yolk sac is a small, round embryonic structure that develops during the earliest weeks of pregnancy. It is one of the first structures visible on ultrasound, typically detectable from around the 5th to 6th week of pregnancy via transvaginal ultrasound. Located outside the embryo but connected to it via the vitelline duct, the yolk sac plays several critical roles before the placenta is fully established.

Functions of the Yolk Sac

Although the human yolk sac is relatively small compared to other species, it performs key functions during early embryonic development:

  • Nutrient transfer: Before the placenta is fully formed, the yolk sac supplies the embryo with essential nutrients.
  • Blood cell formation (hematopoiesis): The yolk sac is the first site of blood cell production, before this function is taken over by the liver and bone marrow.
  • Organ development: Parts of the yolk sac contribute to the formation of the digestive tract, respiratory system, and other internal organs.
  • Germ cell origin: The earliest precursors of reproductive cells (eggs and sperm) originate in the wall of the yolk sac.

The Yolk Sac on Ultrasound

On transvaginal ultrasound, the yolk sac appears as a small, bright, ring-shaped structure, typically measuring between 3 and 6 millimeters in diameter. Its presence confirms an intrauterine pregnancy and is an important marker of early embryonic viability.

Normal Findings

A well-defined, round yolk sac with a diameter of approximately 3–5 mm is considered a normal ultrasound finding in early pregnancy and is a reassuring sign of a healthy, developing pregnancy.

Abnormal Findings

Changes in the appearance or size of the yolk sac may indicate pregnancy complications:

  • Enlarged yolk sac (greater than 6–7 mm): May be associated with an increased risk of miscarriage or chromosomal abnormalities.
  • Absent or very small yolk sac: May suggest a non-viable or failing pregnancy.
  • Irregular shape: Can also be associated with developmental abnormalities.

It is important to note that a single abnormal yolk sac finding is not conclusive on its own. Follow-up ultrasounds and clinical evaluation are always necessary before drawing conclusions.

Regression of the Yolk Sac

As the pregnancy progresses, the placenta gradually takes over the nutritional and metabolic functions previously performed by the yolk sac. By approximately 10 to 12 weeks of gestation, the yolk sac regresses and is no longer visible on ultrasound. This regression is a normal and expected part of embryonic development.

Clinical Significance

Assessment of the yolk sac is a key component of first-trimester pregnancy evaluation. Obstetricians and specialists in prenatal diagnosis use yolk sac visualization to:

  • Confirm the presence of a viable intrauterine pregnancy.
  • Help rule out ectopic pregnancy (pregnancy outside the uterus).
  • Identify early signs of developmental complications.
