What Is the Histamine Tolerance Protein?

The term histamine tolerance protein refers to proteins in the human body that play a central role in the breakdown, regulation, and tolerance of histamine. Histamine is a biogenic amine that occurs naturally in the body and is found in many foods. It is involved in numerous physiological processes, including immune responses, gastric acid regulation, and neurotransmitter functions.

An individual´s histamine tolerance depends largely on how efficiently specific proteins – primarily enzymes – can break down histamine in the body. When this capacity is impaired, a condition known as histamine intolerance can develop.

Key Proteins in Histamine Regulation

Diamine Oxidase (DAO)

Diamine oxidase (DAO) is the most important enzyme protein responsible for breaking down histamine absorbed through food. It is produced mainly in the intestinal mucosa, the placenta, and the kidneys. DAO catalyzes the oxidative deamination of histamine, rendering it harmless. A deficiency or reduced activity of DAO is considered the primary cause of histamine intolerance.

Histamine N-Methyltransferase (HNMT)

Histamine N-methyltransferase (HNMT) is another key enzyme that breaks down histamine inside cells. Unlike DAO, it acts mainly in the intracellular space and is especially active in the brain, liver, and bronchi. It methylates histamine, thereby inactivating it.

Other Involved Proteins

In addition to DAO and HNMT, other proteins are involved in histamine regulation:

  • Histamine receptors (H1–H4): These membrane proteins determine how sensitive cells are to histamine. Changes in receptor density or sensitivity can influence histamine tolerance.
  • MAO (Monoamine oxidase): This enzyme plays a minor role in histamine breakdown and is more relevant for other biogenic amines.

Causes of Impaired Histamine Tolerance

Reduced activity of histamine-degrading proteins can have various causes:

  • Genetic variants (polymorphisms) in the DAO or HNMT genes
  • Gastrointestinal disorders (e.g., inflammatory bowel disease)
  • Certain medications that inhibit DAO (e.g., alcohol, some antibiotics, pain relievers)
  • Nutrient deficiencies (e.g., vitamin B6, copper, and vitamin C as cofactors of DAO)

Symptoms of Reduced Histamine Tolerance

When histamine-degrading proteins are insufficiently active, the following symptoms may occur after consuming histamine-rich foods:

  • Headaches and migraines
  • Skin reactions such as redness, itching, or hives
  • Gastrointestinal complaints (bloating, diarrhea, abdominal pain)
  • Heart palpitations or drop in blood pressure
  • Runny or stuffy nose
  • Asthma-like symptoms

Diagnosis and Treatment

The diagnosis of histamine intolerance is based on medical history, a food diary, and laboratory evidence of reduced DAO activity in the blood. A low-histamine diet is the most important therapeutic measure. In addition, DAO enzyme supplements can be taken to supply the missing protein externally. Antihistamines may relieve symptoms but do not address the underlying cause.
