Uric acid diagnostics refers to the medical assessment of uric acid levels in the body. Uric acid is a breakdown product of purines, which are found in many foods and are generated during cell metabolism. Disruptions in uric acid regulation can lead to several conditions, making this diagnostic approach essential in internal medicine, rheumatology, and nephrology.
Methods of Uric Acid Diagnostics
Uric Acid in Blood (Serum)
The most common method is measuring uric acid concentration in blood serum. A small blood sample is taken and analyzed in the laboratory. Normal reference ranges are generally:
- Men: 3.4 – 7.0 mg/dL (200 – 420 µmol/L)
- Women: 2.4 – 6.0 mg/dL (140 – 360 µmol/L)
Values above these thresholds are referred to as hyperuricemia (elevated uric acid levels), which is a key risk factor for developing gout.
Uric Acid in 24-Hour Urine
The 24-hour urine collection captures all urine produced over one day to measure the total amount of uric acid excreted. This method reveals whether the body is overproducing uric acid or the kidneys are excreting too little. Normal daily excretion is approximately 250 – 750 mg of uric acid.
Spot Urine Sample
In certain clinical situations, a single spot urine sample can be used to calculate the uric acid-to-creatinine ratio. This method is less burdensome for the patient but also less precise than a 24-hour collection.
When Is Uric Acid Diagnostics Used?
Uric acid testing is ordered in a variety of clinical scenarios:
- Suspected gout: Sudden, severe joint pain (typically in the big toe), swelling, and redness.
- Monitoring gout treatment: Checking the effectiveness of uric acid-lowering medications.
- Kidney disease: Elevated uric acid can contribute to kidney stone formation.
- Metabolic syndrome: High uric acid levels often co-occur with obesity, high blood pressure, and elevated blood lipids.
- Cancer therapy monitoring: During tumor lysis syndrome, uric acid can rise sharply.
- Chronic kidney insufficiency: Reduced kidney function commonly leads to elevated uric acid levels.
Clinical Relevance and Interpretation
An elevated uric acid level does not necessarily cause symptoms. Many individuals with asymptomatic hyperuricemia never experience a gout attack. However, persistently elevated levels remain a risk factor for gout, kidney stones, and potentially cardiovascular disease. Conversely, a low uric acid level (hypouricemia) is less common but may occur in certain kidney disorders or following specific medications.
Results should always be interpreted alongside clinical symptoms, dietary history, and other laboratory parameters such as creatinine and the glomerular filtration rate (GFR).
Factors Influencing Uric Acid Levels
Several factors can affect uric acid levels and should be considered during diagnostics:
- Diet: Purine-rich foods such as organ meats, seafood, legumes, and alcohol (especially beer) raise uric acid levels.
- Medications: Diuretics, low-dose aspirin, and ciclosporin can increase uric acid. Allopurinol and febuxostat lower it.
- Kidney function: Reduced kidney function decreases uric acid excretion.
- Body weight: Obesity is associated with higher uric acid levels.
- Genetics: Hereditary factors influence both production and excretion of uric acid.
