What is Phlebitis?

Phlebitis is an inflammation of the wall of a vein. It can affect superficial veins (superficial phlebitis) or deep veins. The superficial form is more common and is generally less dangerous than deep vein inflammation. When phlebitis is accompanied by a blood clot, the condition is referred to as thrombophlebitis.

Causes

Phlebitis can be triggered by a variety of factors:

  • Mechanical irritation: For example, from intravenous catheters, injections, or surgical procedures
  • Varicose veins: Veins already weakened by varicosities are more susceptible to inflammation
  • Infections: Bacterial pathogens can cause septic phlebitis
  • Chemical irritation: Certain medications or infusion solutions can irritate the vein wall
  • Clotting disorders: Conditions that affect blood coagulation increase the risk
  • Prolonged immobility: Extended periods of sitting or lying down promote the development of phlebitis

Symptoms

The typical signs of phlebitis include:

  • Redness along the affected vein
  • Local warmth and swelling
  • Pain or tenderness to the touch
  • Hardening of the tissue above the vein (a palpable, cord-like thickening)
  • Mild fever (in inflammatory or infectious forms)

Diagnosis

Diagnosis is usually made clinically through physical examination and patient history. The following tests may also be used:

  • Ultrasound (duplex sonography): To assess the vein and rule out deep vein thrombosis
  • Blood tests: Inflammatory markers such as CRP and white blood cell count, and D-dimer levels to exclude thrombosis
  • Blood cultures: In suspected cases of septic phlebitis

Treatment

Treatment depends on the underlying cause and the severity of the phlebitis:

Conservative Measures

  • Cooling and elevation of the affected area
  • Compression bandages or compression stockings
  • Anti-inflammatory medications (e.g., diclofenac gel or tablets)
  • Topical heparin-containing gels or creams

Medication-Based Treatment

  • Low-molecular-weight heparin or direct oral anticoagulants for extensive cases or high thrombosis risk
  • Antibiotics in cases of bacterial (septic) phlebitis

Surgical Interventions

  • Removal or sclerotherapy of affected varicose veins in recurrent phlebitis

Complications

In most cases, superficial phlebitis resolves without serious consequences. However, possible complications include:

  • Spread to deep veins, with the risk of deep vein thrombosis (DVT)
  • Pulmonary embolism if a blood clot detaches and travels to the lungs
  • Chronic venous insufficiency
  • Sepsis in untreated bacterial phlebitis

Prevention

The following measures can help prevent phlebitis:

  • Regular physical activity and avoiding prolonged immobility
  • Adequate fluid intake
  • Wearing compression stockings in high-risk individuals
  • Careful handling of intravenous catheters in clinical settings
  • Treatment of varicose veins
