Things worth knowing about "Stereotaxy"

Stereotaxy is a precise neurosurgical technique that uses a three-dimensional coordinate system to accurately target specific areas within the brain.

What is Stereotaxy?

Stereotaxy (also called stereotaxis or stereotactic surgery) is a highly precise medical technique used in neurosurgery and radiation therapy. By applying a three-dimensional coordinate system, physicians can accurately target specific locations within the brain or other body regions with minimal invasiveness. The term derives from the Greek words stereos (solid, three-dimensional) and taxis (arrangement, order).

How Does Stereotaxy Work?

Before the procedure, detailed imaging – typically magnetic resonance imaging (MRI) or computed tomography (CT) – is used to map the target area with millimeter precision. Based on these images, a three-dimensional coordinate plan is calculated. There are two main approaches:

  • Frame-based stereotaxy: A rigid metal frame is attached to the skull to serve as a fixed reference system throughout the procedure.
  • Frameless stereotaxy (neuronavigation): Modern image-guided systems allow precise navigation without a rigid frame, using skin markers or implanted reference points instead.

Clinical Applications

Stereotaxy is used across a wide range of medical indications:

  • Stereotactic biopsy: Collection of tissue samples from deep brain structures to diagnose tumors, inflammation, or other abnormalities.
  • Deep brain stimulation (DBS): Implantation of electrodes to treat Parkinson disease, tremor, and other movement disorders.
  • Stereotactic radiosurgery: Highly focused radiation treatment of tumors or vascular malformations (e.g., using Gamma Knife or CyberKnife), without a physical incision.
  • Stereotactic body radiation therapy (SBRT/SRS): Fractionated radiation for brain or body metastases.
  • Functional neurosurgery: Treatment of epilepsy, chronic pain, or psychiatric disorders through targeted interventions.

Preparation and Procedure

Preparation typically includes detailed imaging studies and careful planning by an interdisciplinary team of neurosurgeons, radiologists, and medical physicists. Depending on the method used, the procedure may be performed under general anesthesia or local anesthesia. Advanced navigation systems allow real-time monitoring throughout the intervention.

Benefits and Risks

Compared to conventional open surgery, stereotaxy offers several advantages:

  • Minimally invasive approach with very small access routes
  • Highest precision in targeting deep structures
  • Reduced risk to surrounding healthy tissue
  • Shorter hospital stays and faster recovery times

However, as with any medical procedure, there are associated risks, including:

  • Bleeding or infection at the intervention site
  • Neurological deficits due to injury of adjacent structures
  • Insufficient therapeutic effect if the target point is inaccurately placed

Significance in Modern Medicine

Stereotaxy has revolutionized neurosurgery and radiation oncology. By combining advanced imaging, computer-assisted planning, and highly precise instruments, procedures that were once impossible or extremely high-risk can now be performed safely and effectively. It is considered a standard technique in specialized neurosurgical centers worldwide.
