What are basophilic granulocytes?

Basophilic granulocytes, commonly referred to as basophils, are a subtype of white blood cells (leukocytes). They are the rarest cells found in the bloodstream, typically making up only 0.5–1% of all leukocytes. Despite their low numbers, basophils play a vital role in the immune system, particularly in allergic reactions and inflammatory processes.

The value basophils absolute indicates exactly how many of these cells are present per microliter (µl) of blood. Unlike the percentage value, the absolute count provides a more direct measure of the actual cell number in the bloodstream.

Normal values

The reference range for basophils in adults is typically between 0 and 100 cells per microliter (µl) of blood, which corresponds to approximately 0.02–0.1 × 10⁹/l. Exact reference values may vary slightly between laboratories.

Causes of elevated basophil counts (basophilia)

An elevated absolute basophil count is known as basophilia. Possible causes include:

  • Allergic conditions: e.g., bronchial asthma, hay fever, food allergies
  • Chronic inflammatory diseases: e.g., ulcerative colitis, rheumatoid arthritis
  • Myeloproliferative disorders: e.g., chronic myeloid leukemia (CML), polycythemia vera
  • Hypothyroidism (underactive thyroid)
  • Infectious diseases: certain viral infections or parasitic infestations
  • Iron deficiency anemia

Causes of decreased basophil counts (basopenia)

Very low or undetectable basophil levels are referred to as basopenia. Since basophils are naturally rare in the blood, basopenia can be clinically difficult to evaluate. Possible causes include:

  • Acute infections or inflammatory states
  • Hyperthyroidism (overactive thyroid)
  • Long-term corticosteroid therapy
  • Physiological stress responses
  • Pregnancy

Function of basophils

Basophils contain granules inside their cytoplasm that release important chemical mediators upon activation. The most significant of these include:

  • Histamine: triggers allergic symptoms such as itching, skin redness, and swelling
  • Heparin: a natural anticoagulant
  • Leukotrienes: signaling molecules that amplify inflammatory responses

Basophils play a central role in IgE-mediated allergic reactions (Type I hypersensitivity) and work closely with mast cells, which are located in body tissues.

Diagnosis and clinical relevance

The absolute basophil count is determined as part of a complete blood count with differential (CBC with differential). When interpreting results, the overall picture of all blood values, clinical symptoms, and medical history must always be taken into account.

An isolated elevated basophil count is generally not sufficient for a diagnosis on its own. Abnormal values should always be discussed with a physician, who may initiate further investigations if necessary, such as a bone marrow biopsy if a hematological disorder is suspected.
