Things worth knowing about "Birth Companionship"

Birth companionship refers to the professional and emotional support provided to a person giving birth before, during, and after labour by midwives or other qualified caregivers.

What is Birth Companionship?

Birth companionship encompasses all forms of professional, emotional, and physical support offered to a pregnant or labouring person before, during, and after childbirth. It is primarily provided by midwives, but may also involve birth companions known as doulas, obstetricians, and other healthcare professionals. The goal is to promote a safe, dignified, and self-determined birth experience.

Forms of Birth Companionship

Midwifery Care

The midwife is the central professional in birth companionship. Midwives provide antenatal care, monitor the progress of labour, support pain management, and care for both mother and newborn during the postnatal period. In Germany, every person giving birth has a legal right to midwifery care.

Doula Support

A doula is a non-medical birth companion who offers continuous emotional, informational, and physical support. A doula does not replace a midwife but complements medical care by providing one-on-one encouragement and advocacy throughout the labour process.

Obstetric Medical Care

Obstetricians and gynaecologists specialising in obstetrics take primary responsibility in cases of high-risk pregnancies, complications, or surgical deliveries such as caesarean sections.

Responsibilities in Birth Companionship

  • Monitoring labour progress and foetal heart rate (cardiotocography, CTG)
  • Supporting breathing techniques and relaxation
  • Advising on pain relief options, including epidural anaesthesia
  • Promoting informed decision-making and advocating for the birthing person
  • Initial care of the newborn
  • Supporting breastfeeding and postnatal recovery

Birth Companionship Across Different Birth Settings

Birth companionship is available regardless of the chosen birth setting. Births may take place in hospitals, birth centres, or at home. Each setting has specific advantages and considerations that should be discussed with a midwife in advance. Hospital settings provide immediate access to interdisciplinary emergency teams when needed.

Benefits of Continuous Birth Companionship

Research demonstrates that continuous one-to-one support during labour is associated with positive outcomes, including shorter labour duration, reduced use of pain medication, lower caesarean section rates, and higher satisfaction among birthing people. The World Health Organization (WHO) explicitly recommends continuous support from a qualified professional during labour.

When Is Specialised Birth Companionship Needed?

Certain circumstances call for more intensive or specialised birth companionship, including:

  • Preterm labour (before 37 weeks of gestation)
  • Multiple pregnancies (e.g., twins)
  • Pre-existing maternal health conditions (e.g., diabetes, hypertension)
  • Psychological stress or previous traumatic birth experiences
  • Abnormal foetal presentation (e.g., breech position)
