What is Clarithromycin?

Clarithromycin is an antibiotic belonging to the macrolide class of drugs. It is a semi-synthetic derivative of erythromycin and has been widely used in clinical medicine since the 1990s. The drug is effective against a broad range of gram-positive and gram-negative bacteria and is generally well tolerated by most patients.

Indications – When is Clarithromycin Used?

Clarithromycin is prescribed for a variety of bacterial infections. Common indications include:

  • Respiratory tract infections: bronchitis, community-acquired pneumonia, sinusitis, and pharyngitis
  • Skin and soft tissue infections: mild to moderate bacterial skin infections
  • Helicobacter pylori eradication: as part of triple therapy for the treatment of peptic ulcers
  • Mycobacterial infections: particularly infections caused by Mycobacterium avium complex (MAC) in immunocompromised patients
  • Otitis media: middle ear infections, especially in children

Mechanism of Action

Clarithromycin inhibits bacterial protein synthesis by reversibly binding to the 50S subunit of the bacterial ribosome. This blocks the translocation of the growing peptide chain, preventing bacterial replication. The drug primarily acts in a bacteriostatic manner (inhibiting growth), but may exert bactericidal (bacteria-killing) effects at higher concentrations. Clarithromycin also demonstrates anti-inflammatory properties, which may be beneficial in the management of chronic respiratory conditions.

Dosage and Administration

Clarithromycin is available as standard tablets, extended-release tablets, and oral suspension (for pediatric use). The typical adult dose is 250 to 500 mg twice daily, with higher doses used for more severe infections. Treatment duration is generally 7 to 14 days depending on the type and severity of infection. Standard tablets can be taken with or without food, while extended-release tablets should be taken with a meal.

Special Patient Populations

  • Renal impairment: Dose adjustment is required in patients with severely reduced kidney function.
  • Hepatic disease: Caution is advised in patients with severe liver disease.
  • Pregnancy and breastfeeding: Use should only be considered after careful assessment of benefits versus risks.

Side Effects

Clarithromycin is generally well tolerated. The most commonly reported side effects include:

  • Gastrointestinal complaints: nausea, vomiting, diarrhea, abdominal pain
  • Altered taste sensation (metallic or bitter taste)
  • Headache
  • Sleep disturbances

Rare but more serious adverse effects include:

  • Hepatotoxicity, including drug-induced hepatitis
  • QT interval prolongation (a change in heart rhythm) – particularly relevant when taken alongside other cardioactive drugs
  • Allergic reactions, including rash or anaphylaxis (rare)
  • Clostridioides difficile-associated diarrhea (pseudomembranous colitis)

Drug Interactions

Clarithromycin is a potent inhibitor of the liver enzyme CYP3A4 and can therefore significantly increase plasma concentrations of many co-administered drugs. Clinically important interactions include:

  • Statins (increased risk of muscle damage – rhabdomyolysis)
  • Anticoagulants such as warfarin (increased bleeding risk)
  • Immunosuppressants such as ciclosporin or tacrolimus
  • Antiarrhythmic drugs (increased risk of cardiac arrhythmias)

Antibiotic Resistance

As with all antibiotics, the development of antibiotic resistance is a growing concern with clarithromycin. Bacteria can acquire resistance through mutations in the ribosomal binding site or via efflux pump mechanisms. Responsible use – meaning use only for confirmed or strongly suspected bacterial infections, at the correct dose and for the recommended duration – is therefore essential to preserve the effectiveness of this antibiotic.
