What Is Vancomycin?

Vancomycin is a glycopeptide antibiotic originally derived from the soil bacterium Amycolatopsis orientalis. It is considered one of the most important last-resort antibiotics in modern medicine, primarily used to treat severe infections caused by gram-positive bacteria that are no longer responsive to other antibiotics.

Mechanism of Action

Vancomycin works by inhibiting bacterial cell wall synthesis. It binds specifically to the terminal end of cell wall precursor molecules (D-alanyl-D-alanine residues), preventing these building blocks from being incorporated into the growing cell wall. Without an intact cell wall, bacteria cannot survive and are destroyed. Vancomycin acts exclusively against gram-positive bacteria, as gram-negative bacteria possess an outer membrane that blocks the drug from entering.

Indications

Vancomycin is indicated for:

  • MRSA infections (methicillin-resistant Staphylococcus aureus) – one of the most common indications
  • Severe skin and soft tissue infections caused by gram-positive organisms
  • Endocarditis (infection of the heart valves) caused by staphylococci or enterococci
  • Central nervous system infections such as bacterial meningitis
  • Clostridioides difficile infections of the gastrointestinal tract (oral administration)
  • Bone infections (osteomyelitis) and joint infections
  • Severe infections in patients with penicillin allergy

Administration and Dosage

Vancomycin can be administered in two main ways:

  • Intravenously (IV): The most common route for systemic infections. The infusion must be administered slowly over at least 60 minutes to minimize adverse effects.
  • Orally: Used exclusively for intestinal infections (e.g., Clostridioides difficile), as the drug is poorly absorbed from the gastrointestinal tract.

Dosage is individualized based on body weight, kidney function, and infection severity. Regular blood level monitoring (therapeutic drug monitoring) is essential to ensure safe and effective dosing throughout therapy.

Side Effects

Vancomycin may cause the following side effects:

  • Nephrotoxicity: Kidney damage, particularly at high doses or when used alongside other nephrotoxic drugs
  • Ototoxicity: Damage to hearing structures, which may result in tinnitus or hearing loss
  • Red Man Syndrome: Flushing and redness of the face, neck, and upper body caused by too-rapid infusion – a histamine-mediated reaction rather than a true allergic response
  • Hypersensitivity reactions and skin rashes
  • Local inflammation at the infusion site (phlebitis)

Resistance

Vancomycin-resistant enterococci (VRE) represent a growing challenge in healthcare settings. These bacteria have altered their cell wall precursor molecules so that vancomycin can no longer bind effectively. The rise of VRE and other resistant organisms underscores the critical importance of responsible and targeted use of last-resort antibiotics.

Important Notes

Vancomycin is a prescription-only last-resort antibiotic that must only be used under medical supervision following targeted diagnostic testing. It is not available for self-medication. Regular monitoring of kidney function and drug blood levels is mandatory throughout treatment to prevent serious complications.
