What is Perenterol?

Perenterol is a pharmacy-only medicinal product containing the probiotic yeast Saccharomyces boulardii as its active ingredient. It is used for the prevention and treatment of diarrheal diseases. Perenterol is available in various forms including capsules, powder, and sachets, and is suitable for both adults and children (e.g., Perenterol junior).

Mechanism of Action

Saccharomyces boulardii is a naturally occurring, non-pathogenic yeast that does not belong to the normal gut flora and is completely eliminated from the intestine after the treatment is stopped. It works through several mechanisms:

  • Inhibition of pathogens: Saccharomyces boulardii produces proteases (enzymes) that break down toxins from diarrhea-causing organisms such as Clostridium difficile and enterotoxigenic Escherichia coli.
  • Strengthening the intestinal barrier: The yeast stimulates the production of secretory immunoglobulin A (sIgA), thereby reinforcing the protective function of the intestinal mucosa.
  • Regulation of gut flora: It supports the balance of the natural gut microbiome, particularly after antibiotic therapy.
  • Anti-inflammatory effects: Saccharomyces boulardii can reduce inflammatory processes in the gut and enhance local immune defenses.

Indications

Perenterol is indicated for the following conditions:

  • Acute infectious diarrhea (e.g., traveler's diarrhea)
  • Antibiotic-associated diarrhea (diarrhea as a side effect of antibiotic treatment)
  • Prevention of traveler's diarrhea
  • Inflammatory bowel disease (as a supportive measure)
  • Diarrhea in infants and children (Perenterol junior)

Dosage and Administration

The exact dosage of Perenterol depends on the age, body weight, and severity of the condition. General guidelines include:

  • Adults: Typically 1-2 capsules or sachets (250 mg each) once or twice daily.
  • Children (Perenterol junior): Dosed according to body weight, usually 1 sachet (250 mg) once or twice daily.
  • Should be taken with sufficient liquid. The powder can be stirred into lukewarm water or juice.
  • For acute diarrhea, the recommended duration of treatment is generally 5-10 days.

Side Effects

Perenterol is generally well tolerated. Known side effects include:

  • Common: Flatulence, bloating, mild abdominal discomfort at the start of treatment
  • Rare: Allergic reactions to yeast components (especially in individuals with known yeast allergy)
  • Very rare: Fungemia (entry of yeast into the bloodstream) in severely immunocompromised patients

Contraindications and Warnings

Perenterol should not be used in:

  • Patients with known allergy to Saccharomyces boulardii or other yeasts
  • Severely immunocompromised patients (e.g., HIV/AIDS, chemotherapy, organ transplant recipients) -- medical advice should be sought before use
  • Patients with central venous catheters (increased risk of fungemia)

During pregnancy and breastfeeding, Perenterol should only be used after consulting a healthcare professional.
