Definition and Anatomy

The anterior carpal region (Latin: Regio carpalis anterior) refers to the palmar (flexor) side of the wrist. It is located between the distal forearm and the hand and forms the anterior counterpart to the posterior carpal region (dorsal side).

This region encompasses the skin and subcutaneous tissue on the palmar aspect of the wrist, the underlying flexor tendons, major blood vessels, and two important nerves: the median nerve and the ulnar nerve.

Key Anatomical Structures

Tendons

Flexor carpi radialis tendon: runs along the radial side and serves as a landmark for pulse palpation.
Flexor carpi ulnaris tendon: runs along the ulnar side, guiding the ulnar nerve.
Palmaris longus tendon: lies centrally and is absent in approximately 15% of individuals.
Tendons of the superficial and deep finger flexors pass through the carpal tunnel.

Blood Vessels

Radial artery: runs along the radial side and is the standard site for pulse measurement at the wrist.
Ulnar artery: runs along the ulnar side, accompanied by the ulnar nerve.

Nerves

Median nerve: passes through the carpal tunnel, providing motor and sensory innervation to the radial palm and fingers.
Ulnar nerve: travels through the Guyon canal (ulnar canal) on the medial side of the wrist.

Clinical Relevance

Carpal Tunnel Syndrome

Carpal tunnel syndrome is the most common condition affecting the anterior carpal region. It occurs when the median nerve is compressed within the carpal tunnel, causing numbness, tingling, and pain in the hand and fingers. Diagnosis is clinical and electrophysiological; treatment may be conservative or surgical.

Guyon Canal Syndrome

Compression of the ulnar nerve within the Guyon canal leads to Guyon canal syndrome, characterized by sensory disturbances on the ulnar side of the hand and weakness of intrinsic hand muscles.

Traumatic Injuries

Lacerations and fractures in the anterior carpal region can simultaneously damage tendons, vessels, and nerves, often requiring microsurgical repair. Common injuries include the scaphoid fracture and ligamentous injuries of the wrist.

Pulse Measurement

The radial artery in the anterior carpal region is the standard site for clinical pulse palpation, arterial blood pressure measurement, and arterial blood gas sampling.

Examination of the Anterior Carpal Region

Clinical examination includes inspection (swelling, bruising, scars), palpation (tenderness, pulse), range of motion assessment, and neurological tests such as the Tinel test and the Phalen test to detect carpal tunnel syndrome. Imaging modalities such as ultrasound and MRI provide additional diagnostic information.
