Arthrography is a specialized diagnostic imaging technique in which a contrast agent is injected directly into a joint to enhance the visibility of its internal structures. This allows for detailed assessment of components such as cartilage, ligaments, tendons, menisci, and the joint capsule. Arthrography is most commonly performed in combination with Magnetic Resonance Imaging (MRI) or Computed Tomography (CT), referred to as MR arthrography and CT arthrography, respectively.
Arthrography is typically used when standard imaging techniques such as conventional MRI or X-ray do not provide sufficient detail. Common indications include:
Before the procedure, the skin over the affected joint is disinfected and a local anesthetic is applied. Under imaging guidance -- typically fluoroscopy or ultrasound -- a thin needle is carefully inserted into the joint space. The contrast agent is then injected: an iodine-based agent for CT arthrography or a gadolinium-based agent for MR arthrography. Following the injection, the joint is examined in detail using MRI or CT imaging.
Direct arthrography involves injecting the contrast agent directly into the joint, while indirect arthrography involves intravenous administration of contrast material, which then accumulates in the joint via the bloodstream. Direct arthrography generally provides higher image quality and diagnostic accuracy.
Arthrography is considered a safe procedure overall. However, as with any invasive diagnostic method, some risks exist:
Patients with a known allergy to contrast agents or impaired kidney function should inform their physician prior to the examination, as special precautions may be necessary.
Compared to standard MRI without contrast enhancement, arthrography provides significantly improved visualization of fine intra-articular structures. Small tears in cartilage, ligaments, or the joint capsule can be detected more reliably. This makes arthrography a valuable tool in orthopedic and sports medicine diagnostics, particularly for pre-surgical planning.
