What is the BASDAI?
The BASDAI (Bath Ankylosing Spondylitis Disease Activity Index) is a standardized, patient-completed questionnaire that measures disease activity in ankylosing spondylitis (also known as Bechterew's disease). Developed in the 1990s in Bath, United Kingdom, it has become one of the most widely used tools for evaluating this condition worldwide.
The BASDAI allows clinicians to systematically capture the subjective symptoms reported by patients and to objectively compare treatment outcomes over time.
Structure and Administration
The questionnaire consists of 6 questions relating to the past week. Patients rate their symptoms on a numerical scale from 0 to 10:
- Level of fatigue and tiredness
- Level of spinal pain
- Level of pain or swelling in joints (excluding the neck and back)
- Level of discomfort at tender areas (enthesitis)
- Level of morning stiffness (severity)
- Duration of morning stiffness
The last two questions on morning stiffness are averaged together and then combined with the other four questions to calculate an overall score.
Calculating the BASDAI Score
The BASDAI score is calculated using the following formula:
BASDAI = (Question 1 + Question 2 + Question 3 + Question 4 + Mean of Questions 5 and 6) / 5
The result ranges between 0 and 10. A higher score indicates greater disease activity.
Clinical Significance and Interpretation
A BASDAI score of ≥ 4 is considered indicative of high disease activity and, in accordance with ASAS/EULAR guidelines, may justify initiating treatment with biologic therapies such as TNF-alpha inhibitors or IL-17 inhibitors.
- 0–3: Low to moderate disease activity
- 4–6: Moderate to high disease activity
- 7–10: Very high disease activity
The BASDAI is used regularly throughout the course of treatment to monitor therapeutic response and guide clinical decision-making.
Advantages and Limitations
A key advantage of the BASDAI is its simplicity: it can be completed by the patient themselves without the need for laboratory tests or imaging. However, it captures only subjective symptoms and does not account for objective inflammatory markers such as CRP (C-reactive protein) or imaging findings. For this reason, the BASDAI is often used alongside other assessment tools such as the ASDAS (Ankylosing Spondylitis Disease Activity Score), which also incorporates laboratory values.
