Definition and Anatomy

The ductuli prostatici (singular: ductulus prostaticus), also known as prostatic ducts or prostatic ductules, are the small excretory channels of the male prostate gland. The prostate consists of numerous tubuloalveolar glands whose secretions are delivered into the urethra through these fine ducts. The openings of the ductuli prostatici are located predominantly on either side of the colliculus seminalis (verumontanum) within the prostatic urethra (pars prostatica urethrae). The total number of these ducts is estimated to be between 15 and 30.

Function

The primary function of the ductuli prostatici is to transport the secretion produced by the prostatic glandular cells into the urethra. Prostatic secretion accounts for approximately 20 to 30 percent of the total ejaculate volume and contains several important components:

  • Prostate-specific antigen (PSA): an enzyme that contributes to the liquefaction of semen
  • Citric acid: creates an optimal environment for sperm cells
  • Zinc: has antimicrobial properties and helps stabilize sperm DNA
  • Acid phosphatase: an enzyme relevant to sperm maturation

By ensuring the proper delivery of prostatic secretion into the ejaculatory pathway, the ductuli prostatici play a significant role in male fertility.

Histological Structure

The ductal epithelium lining the ductuli prostatici is composed of two cell layers: luminal (secretory) cells and basal cells. The luminal cells are columnar to cuboidal in shape and characteristically express the androgen receptor and PSA. Basal cells serve as a reserve compartment and are involved in tissue regeneration. Scattered neuroendocrine cells are also found within the epithelium, producing regulatory peptides that influence glandular activity.

Clinical Relevance

Prostatitis

Prostatitis (inflammation of the prostate) can lead to obstruction or inflammation of the ductuli prostatici. Blocked ducts may cause stasis of prostatic secretion and contribute to the formation of prostatic calculi (prostatic stones). Common symptoms include pelvic pain, burning during urination, and reduced urinary flow.

Benign Prostatic Hyperplasia (BPH)

In benign prostatic hyperplasia, the enlarged glandular tissue can compress the ductuli prostatici and impair their drainage. This may lead to lower urinary tract symptoms and increase the risk of infection or stone formation.

Prostate Cancer

Prostate cancer most commonly originates in the glandular acini and ductuli prostatici, particularly in the peripheral zone of the prostate. Changes in the ductal epithelium, such as prostatic intraepithelial neoplasia (PIN), are considered potential precursor lesions. Early detection through PSA testing and biopsy is of critical importance.

Ejaculatory Dysfunction and Fertility

Dysfunction of the ductuli prostatici or their opening regions can affect the composition of ejaculate and contribute to male subfertility. Urological evaluation may be required in such cases to identify and treat the underlying cause.

Diagnostics

Due to their small size, the ductuli prostatici cannot be directly visualized in standard imaging modalities such as transrectal ultrasound or MRI. Their function is assessed clinically through prostatic secretion analysis, semen analysis, and serum PSA measurement. Direct histological evaluation is performed as part of a prostate biopsy.
