What Is Early Pregnancy?

Early pregnancy refers to the period from the first day of the last menstrual period through the end of the 12th week of gestation. Medically, this corresponds to the first trimester – the first third of pregnancy. During these initial weeks, the most significant developmental milestones occur: all major organs and body structures are formed in a process known as organogenesis.

From around week 10, the developing baby transitions from being called an embryo to a fetus. The risk of miscarriage is highest during the first trimester and decreases significantly after week 12.

Development During Early Pregnancy

Weeks 1–4: Fertilization and Implantation

After fertilization of the egg by a sperm cell, the resulting fertilized cell – called a zygote – travels through the fallopian tube and implants into the lining of the uterus. This process, known as implantation, occurs approximately 6–10 days after fertilization. The resulting cluster of cells forms the blastocyst, which will later develop into both the embryo and the placenta.

Weeks 5–8: Organ Formation

During this phase, organogenesis begins in earnest. The heart, brain, spinal cord, eyes, and limbs start to develop. The embryonic heart begins beating as early as week 6 and can be detected by ultrasound. By the end of this phase, the embryo measures approximately 1–2 cm in length.

Weeks 9–12: Transition to Fetus

All major organs have now been established and begin to mature. Fingers and toes become distinguishable, and facial features start to form. By the end of week 12, the fetus measures approximately 6–7 cm. The placenta gradually takes over the supply of nutrients and oxygen to the growing baby.

Symptoms of Early Pregnancy

Many women first notice pregnancy due to a missed period. Other common symptoms include:

  • Nausea and vomiting (morning sickness), often triggered by rising levels of the pregnancy hormone hCG
  • Breast tenderness and swelling
  • Fatigue and exhaustion
  • Frequent urination
  • Food aversions or cravings
  • Mild cramping or a pulling sensation in the lower abdomen
  • Mood changes caused by hormonal fluctuations

Diagnosis of Pregnancy

The first indication of pregnancy is usually a positive pregnancy test, which detects the hormone hCG (human chorionic gonadotropin) in the urine. These tests are reliable from the first day of the missed period onwards. Confirmation is provided by a blood test at the doctor and by ultrasound, which can typically detect the embryonic heartbeat from around week 6.

As part of standard antenatal care, first trimester screening is offered between weeks 11 and 14. This combines an ultrasound measurement of the nuchal translucency with blood tests to assess the risk of chromosomal conditions such as Down syndrome (Trisomy 21).

Risks and Complications

The first trimester is the most vulnerable phase of pregnancy. The most common risks include:

  • Miscarriage (spontaneous abortion): Approximately 10–20% of all known pregnancies end in miscarriage during the first trimester, most commonly due to chromosomal abnormalities in the embryo.
  • Ectopic pregnancy: The embryo implants outside the uterus, most often in a fallopian tube. This is a medical emergency requiring immediate treatment.
  • Hyperemesis gravidarum: Severe and persistent nausea and vomiting that can lead to dehydration and significant weight loss.
  • Subchorionic hematoma: A collection of blood between the placenta and the uterine wall, which may cause bleeding and spotting.

Recommendations for a Healthy Early Pregnancy

The following recommendations are important for supporting a healthy early pregnancy:

  • Folic acid: A daily supplement of 400–800 µg of folic acid should ideally begin before conception and continue at least through week 12 to reduce the risk of neural tube defects.
  • Avoiding alcohol and tobacco: Both substances must be strictly avoided during pregnancy, as they can harm embryonic development.
  • Medication safety: Many medications are contraindicated in early pregnancy. Always consult a healthcare provider before taking any medication.
  • Nutrition: A balanced diet rich in iodine, iron, and calcium is recommended. Raw meat, raw fish, and raw eggs should be avoided to reduce the risk of listeriosis and toxoplasmosis.
  • Antenatal check-ups: Regular prenatal appointments are essential for monitoring the health of both the mother and the developing baby.
