What is Enterobiasis?
Enterobiasis (also known as oxyuriasis or pinworm infection) is an intestinal infection caused by the parasitic worm Enterobius vermicularis, commonly called the pinworm or threadworm. It is the most common worm infection worldwide and predominantly affects school-age children and their household contacts. While generally harmless, the infection can significantly impact quality of life due to persistent itching and sleep disturbances.
Causes and Transmission
The causative agent is the small, white pinworm (Enterobius vermicularis), which lives in the human large intestine. Transmission occurs primarily through the fecal-oral route:
- Pinworm eggs are ingested via contaminated hands, bedding, clothing, or surfaces.
- Female worms migrate to the anal region at night to deposit their eggs on the surrounding skin.
- Scratching transfers eggs to the fingernails and fingers, where they can be reingested (autoinfection).
- Spread within families and group settings such as kindergartens and schools is common.
Symptoms
Many infected individuals have no symptoms or only mild complaints. Typical symptoms of enterobiasis include:
- Intense anal itching, especially at night (perianal pruritus)
- Disturbed sleep and restlessness
- Irritability and difficulty concentrating, particularly in children
- Occasional abdominal discomfort or nausea
- Visible small white worms in the stool or around the anus
- In girls: genital itching or vulvovaginitis may occur if worms migrate to the vaginal area
Diagnosis
Enterobiasis is usually diagnosed clinically, but can be confirmed with simple tests:
- Tape test (Scotch-tape test): A piece of transparent adhesive tape is pressed against the anal skin in the morning, before washing or using the toilet. The tape is then examined under a microscope to detect the characteristic pinworm eggs.
- Visual identification: Adult worms may be visible to the naked eye around the anus or in the stool. They are white, thin, and approximately 1 cm in length.
- Standard stool examination is not reliable for diagnosis, as eggs are deposited outside the intestine.
Treatment
Enterobiasis is easily treatable with anthelmintic medications (anti-worm drugs):
- Mebendazole or Pyrantel pamoate: Administered as a single dose, typically as a tablet or oral suspension. A repeat dose after 2 weeks is recommended to prevent reinfection.
- Since the infection spreads easily among household members, all members of the household should be treated simultaneously.
Hygiene Measures
To prevent reinfection and further spread, the following hygiene steps are essential:
- Thorough and frequent handwashing, especially after using the toilet and before eating
- Keep fingernails short and clean them regularly
- Change underwear and bed linen daily during and after treatment
- Wash laundry at a minimum of 60°C (140°F)
- Regularly ventilate and clean bedrooms and living areas
Prognosis
Enterobiasis is a benign condition that can be fully resolved with appropriate treatment and strict hygiene measures. However, reinfection is common, especially in children attending group settings. Serious complications are rare. Consistent adherence to hygiene practices is the most effective way to prevent recurrence.
