What is Fenofibrate?
Fenofibrate is a prescription drug belonging to the class of fibrates. It is used to treat elevated blood lipid levels – in particular high triglycerides and low HDL cholesterol (the so-called good cholesterol). Fenofibrate is commonly used alongside dietary changes and other lipid-lowering therapies to reduce cardiovascular risk.
Indications
Fenofibrate is indicated for:
- Hypertriglyceridaemia: significantly elevated triglyceride levels in the blood
- Mixed dyslipidaemia: a combination of elevated triglycerides, elevated LDL cholesterol, and low HDL cholesterol
- As an add-on therapy to statins when triglyceride levels remain uncontrolled despite statin treatment
It is particularly used in patients with type 2 diabetes or metabolic syndrome, conditions frequently associated with lipid disorders.
Mechanism of Action
Fenofibrate acts as a PPAR-alpha agonist (peroxisome proliferator-activated receptor alpha). By activating this nuclear receptor in the liver, it triggers several lipid-lowering effects:
- Increased oxidation (breakdown) of fatty acids in the liver
- Reduced production of triglyceride-rich lipoproteins (VLDL)
- Enhanced activity of lipoprotein lipase, an enzyme that breaks down triglycerides in the bloodstream
- Increased HDL cholesterol levels through upregulation of apolipoprotein A-I and A-II production
The overall result is a significant reduction in triglycerides (by up to 50%) and a moderate increase in HDL cholesterol.
Dosage
Fenofibrate is available in various formulations and strengths (e.g., 100 mg, 145 mg, 160 mg, 200 mg, 267 mg). The recommended daily dose depends on the specific formulation and should always be taken as directed by a physician. Some preparations are taken once daily with a meal, while others are food-independent. Dose adjustment is required in patients with renal impairment.
Side Effects
Like all medications, fenofibrate can cause side effects. Common and clinically relevant side effects include:
- Gastrointestinal complaints: nausea, abdominal pain, diarrhoea
- Elevated liver enzymes: regular monitoring of liver function is recommended
- Muscle pain (myopathy): in rare cases, this may progress to rhabdomyolysis (severe muscle breakdown), especially when combined with statins
- Pancreatitis: inflammation of the pancreas (rare)
- Gallstones: fenofibrate may promote gallstone formation
- Photosensitivity reactions of the skin
Contraindications and Interactions
Fenofibrate must not be used in patients with:
- Severe hepatic or renal impairment
- Existing gallbladder disease
- Known hypersensitivity to the active substance
Important drug interactions include:
- Statins: increased risk of muscle damage (myopathy)
- Anticoagulants (e.g., warfarin, phenprocoumon): fenofibrate may enhance anticoagulant effects; dose adjustment may be necessary
- Ciclosporin: potential interactions requiring close monitoring
Treatment Context
Fenofibrate is used as part of a comprehensive treatment strategy to reduce cardiovascular risk. In addition to drug therapy, a low-fat diet, regular physical activity, weight reduction, and alcohol restriction are essential components of managing lipid disorders. Regular blood tests (lipid panel, liver function, kidney function) are recommended during therapy.
