What is Heterophoria?
Heterophoria (also known as latent strabismus or latent squint) is a condition in which the eyes tend to deviate from their normal alignment when the visual system is at rest. Unlike manifest strabismus (heterotropia), this deviation is continuously corrected by involuntary tension in the eye muscles, allowing for normal binocular vision in most daily situations. However, when the compensatory mechanisms are overloaded – for example due to fatigue, prolonged screen use, or stress – symptoms may develop.
Causes
Heterophoria can result from several factors, including:
- Innate imbalance in the strength or resting tone of the extraocular muscles
- Uncorrected refractive errors such as myopia (nearsightedness), hyperopia (farsightedness), or astigmatism
- Neurological conditions affecting eye motility
- Prolonged near work, particularly screen-based tasks
- Chronic fatigue or high levels of stress
Types of Heterophoria
Heterophoria is classified according to the direction of the latent deviation:
- Esophoria: Latent inward deviation of one eye
- Exophoria: Latent outward deviation of one eye
- Hypophoria / Hyperphoria: Latent downward or upward deviation of one eye
- Cyclophoria: Latent rotational deviation of one eye around the visual axis
Symptoms
Many people with heterophoria experience no symptoms at all. However, when the capacity for compensation is exceeded, the following symptoms may occur:
- Headaches, especially after reading or screen work
- Eye fatigue and burning or itching eyes
- Temporary blurred or double vision
- Difficulty concentrating while reading
- General tiredness or discomfort
- A feeling of pressure behind the eyes
Diagnosis
Heterophoria is diagnosed by an ophthalmologist or an optometrist using the following methods:
- Cover test (alternating cover test): One eye is covered and the movement of the other eye is observed to reveal a latent deviation.
- Maddox rod / Maddox cross: Specialized optical tests used to measure the degree and direction of heterophoria.
- Prism test: Prisms of varying strength are used to quantify the extent of the deviation.
- Refraction assessment: Examination for refractive errors that may contribute to heterophoria.
Treatment
Not every case of heterophoria requires treatment. Therapy is generally only indicated when the patient experiences symptoms. Available treatment options include:
- Spectacle correction: Correcting underlying refractive errors can reduce the strain on the eye muscles and alleviate symptoms.
- Prismatic lenses: Prisms incorporated into spectacle lenses can optically compensate for the misalignment and relieve muscular tension.
- Orthoptic exercises (vision therapy): Targeted exercises to strengthen the eye muscles and improve binocular coordination.
- Surgical treatment: In rare, severe cases, surgery on the extraocular muscles may be considered.
