What Is a Dental Inlay?
A dental inlay is a precisely crafted tooth restoration used to repair significant damage caused by decay, fracture, or a failing filling. Unlike a conventional direct filling that is applied and shaped directly in the mouth, an inlay is fabricated in a dental laboratory or using CAD/CAM technology and then permanently cemented into the prepared tooth cavity. This process results in an exceptionally accurate fit, long-lasting durability, and a high-quality aesthetic outcome.
Materials
Inlays can be made from several materials depending on clinical needs and patient preferences:
- Ceramic (Porcelain): The most popular choice for visible teeth due to its natural tooth-like appearance and excellent biocompatibility.
- Gold: Highly durable and well-tolerated by oral tissues; typically used in posterior (back) teeth where appearance is less of a concern.
- Composite Resin: A more affordable option with good aesthetics, though slightly less resistant to wear compared to ceramic or gold.
Indications
A dental inlay is recommended when:
- A tooth has been significantly damaged by decay, fracture, or a failing old restoration
- The damage is too extensive for a conventional filling but does not yet require a full dental crown

Treatment Procedure
The inlay procedure typically requires two dental appointments:
First Appointment: Preparation
The dentist removes all decay or the defective filling and carefully shapes the tooth cavity to accommodate the inlay. A precise dental impression or a digital 3D scan of the prepared tooth is then taken and sent to the dental laboratory. A temporary filling protects the tooth in the interim period.
Second Appointment: Placement
Once the custom inlay is ready, the dentist checks its fit, verifies the bite, and permanently bonds it into place using a specialized dental cement.
Advantages and Disadvantages
Advantages
- Extremely precise fit and long lifespan (often 10–20 years or more)
- Superior aesthetic results with ceramic inlays
- Minimal removal of healthy tooth structure

Disadvantages
- Higher cost compared to conventional direct fillings
- Requires at least two dental appointments
- Ceramic inlays may be susceptible to chipping under extreme biting forces
Inlay vs. Onlay vs. Overlay
Closely related to the inlay are the onlay and overlay. While an inlay sits entirely within the cusps of a back tooth, an onlay extends over one or more cusps, and an overlay covers the entire chewing surface. The appropriate restoration type is chosen based on the extent of the tooth damage.
