What is Moxonidine?
Moxonidine is a prescription medication used to treat high blood pressure (arterial hypertension). It belongs to the drug class of centrally acting antihypertensives, specifically the selective imidazoline I1 receptor agonists. By acting in the brain, moxonidine lowers blood pressure without significantly affecting the heart rate.
Mechanism of Action
Moxonidine selectively binds to imidazoline I1 receptors in the rostral ventrolateral medulla of the brainstem. This inhibits the activity of the sympathetic nervous system, reducing the release of noradrenaline. As a result:
- peripheral vascular resistance decreases
- both systolic and diastolic blood pressure are lowered
- the workload on the heart is reduced
Compared to the older agent clonidine, moxonidine has a higher selectivity for imidazoline receptors and fewer effects on alpha-2 adrenoceptors, resulting in a more favorable side effect profile.
Indications
Moxonidine is primarily used for:
- Mild to moderate arterial hypertension
- High blood pressure not sufficiently controlled by other medications (combination therapy)
- Patients with metabolic syndrome or insulin resistance, as moxonidine may also improve insulin sensitivity
Dosage
The standard dosage is 0.2 mg to 0.4 mg once daily, preferably in the morning. In certain cases, the dose may be increased to 0.6 mg per day, divided into two administrations. Tablets should be swallowed whole with sufficient fluid. Dose adjustment is required in patients with impaired kidney function.
Important Dosage Notes
- Moxonidine should not be stopped abruptly, as this may lead to a rebound increase in blood pressure.
- Gradual dose reduction under medical supervision is recommended.
Side Effects
Moxonidine is generally well tolerated. The most common side effects include:
- Dry mouth (most frequent side effect)
- Fatigue or drowsiness
- Dizziness
- Headache
- Sleep disturbances
- Skin reactions (rare)
Serious side effects are rare but may include cardiac arrhythmias or angioedema.
Contraindications
Moxonidine must not be used in patients with:
- Severe heart failure (NYHA class III-IV)
- Sick sinus syndrome or AV block (2nd and 3rd degree)
- Severe renal insufficiency (GFR < 30 ml/min)
- Bradycardia (heart rate < 50 beats per minute)
- Known hypersensitivity to the active substance
- Pregnancy and breastfeeding
Drug Interactions
Moxonidine may interact with other medications:
- Tricyclic antidepressants: may reduce the antihypertensive effect of moxonidine
- Sedatives and alcohol: may enhance the central sedative effect
- Other antihypertensive agents: additive blood pressure lowering effect possible
- Benzodiazepines: increased sedation possible
Treatment Context
Moxonidine is not considered a first-line agent for hypertension according to current European guidelines (ESC/ESH), but it is used as add-on therapy when blood pressure is not adequately controlled. It is particularly suitable for patients in whom beta-blockers or ACE inhibitors are not appropriate, or in the presence of metabolic syndrome.
