Nitrofurantoin is an antibiotic specifically used to treat urinary tract infections (UTIs). It belongs to the nitrofuran class of antibiotics and has been a well-established treatment for bacterial infections of the bladder and lower urinary tract for decades. The medication is available by prescription and is typically taken as a tablet or capsule.
Indications
Nitrofurantoin is primarily used for:
- Uncomplicated urinary tract infections (e.g., cystitis, bladder infection)
- Prophylaxis (prevention) of recurrent UTIs in susceptible patients
It is not suitable for the treatment of kidney infections (pyelonephritis) or complicated UTIs, as drug concentrations in kidney tissue are insufficient.
Mechanism of Action
After oral ingestion, nitrofurantoin is rapidly absorbed and excreted primarily through the kidneys, achieving high concentrations in the urine. Within the urine, the drug is converted by bacterial enzymes into reactive intermediates that:
- Damage bacterial DNA
- Inhibit key bacterial metabolic enzymes
- Disrupt bacterial cell wall synthesis
This multi-target mechanism of action makes the development of resistance comparatively uncommon.
Dosage
Dosage depends on the formulation (immediate-release or modified-release) and the clinical indication:
- Acute cystitis: Typically 50–100 mg, 3–4 times daily for 5–7 days (immediate-release) or 100 mg modified-release twice daily for 5 days
- Long-term prophylaxis: 50–100 mg once nightly
Nitrofurantoin should always be taken with food to improve absorption and reduce the risk of gastrointestinal side effects.
Side Effects
Like all medications, nitrofurantoin can cause side effects. Common and clinically relevant adverse effects include:
- Gastrointestinal complaints: Nausea, vomiting, diarrhea
- Urine discoloration: Dark yellow to brown urine (harmless)
- Allergic reactions: Skin rash, itching
- Pulmonary toxicity: Rarely, with long-term use, cough, shortness of breath, or pulmonary fibrosis may occur
- Hepatotoxicity: Very rare but possible with prolonged therapy
- Peripheral neuropathy: Tingling or numbness in the hands and feet with extended use
Contraindications
Nitrofurantoin must not be used in:
- Severe renal impairment (eGFR < 30 ml/min), as impaired renal excretion can lead to toxic drug accumulation
- Pregnancy from week 38 onwards (risk to the newborn)
- Infants under one month of age
- Patients with known G6PD deficiency (glucose-6-phosphate dehydrogenase deficiency) due to the risk of hemolytic anemia
Drug Interactions
Important interactions with nitrofurantoin include:
- Antacids containing magnesium trisilicate may reduce absorption
- Probenecid and sulfinpyrazone may inhibit renal excretion, leading to elevated plasma levels
- Concurrent use of other nephrotoxic or neurotoxic agents should be avoided
Treatment Context
According to guidelines from the European Association of Urology (EAU) and the Infectious Diseases Society of America (IDSA), nitrofurantoin is considered a first-line treatment for uncomplicated cystitis. A key advantage is its low resistance rate against common urinary pathogens such as Escherichia coli.
