What is Osmolarity?

Osmolarity refers to the total concentration of all dissolved particles (called osmoles) in a solution. It is expressed in the unit mosmol/l (milliosmoles per liter). In clinical medicine, osmolarity is a central parameter for evaluating the water and electrolyte balance of the human body.

A closely related term is osmolality, which describes the concentration of dissolved particles per kilogram of solvent. While technically different, both terms are often used interchangeably in clinical settings.

Physiological Significance

The osmolarity of human blood plasma normally ranges between 280 and 300 mosmol/l. This value is tightly regulated by various mechanisms, particularly through hormones such as ADH (antidiuretic hormone) and kidney function.

The main substances contributing to osmolarity include:

  • Sodium (the most important factor in the extracellular space)
  • Potassium
  • Glucose
  • Urea
  • Proteins and other molecules

Osmosis and Osmotic Pressure

Osmolarity is closely linked to the principle of osmosis -- the movement of water across a semipermeable membrane from an area of lower osmolarity to an area of higher osmolarity. The resulting osmotic pressure is critical for water exchange between cells, tissues, and blood.

Clinical Relevance and Measurement

Osmolarity can be measured in the laboratory either directly using an osmometer (based on freezing point depression) or estimated using a calculation formula:

Calculated osmolarity = 2 × Sodium + Glucose + Urea (all values in mmol/l)

The difference between the measured and calculated osmolarity is called the osmolar gap. An elevated osmolar gap may indicate poisoning (e.g., from methanol or ethylene glycol) or the presence of unusual osmotically active substances.

Disorders of Osmolarity

Hyperosmolarity

Hyperosmolarity (elevated osmolarity, above 300 mosmol/l) means that the concentration of dissolved particles in the blood is too high. Causes include:

  • Dehydration
  • Diabetes mellitus (elevated blood glucose)
  • Certain medications
  • Kidney disease

Hypo-osmolarity

Hypo-osmolarity (reduced osmolarity, below 280 mosmol/l) means that the concentration of dissolved particles is too low. Causes include:

  • Excessive water intake
  • Heart failure or liver disease
  • SIADH (syndrome of inappropriate antidiuretic hormone secretion)

Osmolarity in Clinical Nutrition and Intensive Care

Osmolarity also plays an important role in clinical nutrition. Enteral and parenteral nutritional solutions must have a tolerable osmolarity to avoid damage to the gastrointestinal tract or blood vessels. Solutions with very high osmolarity must be administered via a central venous catheter.

In intensive care settings, plasma osmolarity is regularly monitored to detect and treat disturbances in water and electrolyte balance at an early stage.
