What Is a Percentile?

A percentile is a statistical value that indicates the relative position of an individual measurement within a defined reference population. It expresses what percentage of the comparison group falls below a given value.

For example, if a child is at the 25th percentile for height, this means that 25% of children of the same age and sex are shorter, while 75% are taller. The 50th percentile corresponds to the median – the midpoint of the distribution.

Medical Applications

Percentiles are used across many areas of medicine to place individual measurements into a population context:

Growth assessment in children: Height, weight, and head circumference are plotted on growth charts using percentile curves.
Body Mass Index (BMI): In children and adolescents, BMI is assessed using age- and sex-specific percentile charts.
Laboratory values: Reference ranges for blood and urine tests are often expressed as percentile-based intervals.
Birth weight assessment: Newborn weight is evaluated using percentiles to identify low birth weight or macrosomia.
Blood pressure in children: Blood pressure values are classified using age-, sex-, and height-specific percentile tables.

How Are Percentile Charts Created?

Percentile charts are developed from large population studies in which measurements from representative samples are collected and statistically analyzed. Widely used reference charts include those published by the World Health Organization (WHO) for children aged 0 to 5 years, and national reference charts tailored to specific populations.

Standard charts typically display the following percentile lines: 3rd, 10th, 25th, 50th, 75th, 90th, and 97th percentile. Values below the 3rd or above the 97th percentile are generally considered clinically significant and warrant further medical evaluation.

Interpretation and Clinical Relevance

Individual percentile values must always be interpreted in the context of longitudinal measurements and the overall clinical picture. A child at the 10th percentile is not necessarily unhealthy – what matters most is whether the value remains stable over time or whether a significant downward shift across measurement points is observed (referred to as percentile crossing or percentile drop).

Clinically relevant situations include:

A drop in weight or height across two or more major percentile lines
Values below the 3rd percentile (possible indicator of failure to thrive or growth delay)
Values above the 97th percentile (possible indicator of overweight or hormonal disorders)
Disproportionate development between different parameters (e.g., weight and height diverging significantly)

Percentile vs. Normal Value

A common misconception is that only values near the 50th percentile are considered normal. In reality, any value between the 3rd and 97th percentile is generally regarded as a normal variant. This range covers 94% of all measurements in the reference population.
