What Is Pyelonephritis?

Pyelonephritis, commonly known as a kidney infection or kidney pelvis inflammation, is a bacterial infection affecting the renal pelvis and kidney tissue. It most often occurs when bacteria – frequently Escherichia coli – ascend from the bladder into the kidneys. Pyelonephritis can be acute or chronic and always requires medical treatment.

Typical Symptoms of Pyelonephritis

The symptoms of a kidney infection often develop rapidly and can be quite severe. The most common complaints include:

  • Flank pain: Pain in the back or side, typically one-sided and located below the ribs
  • High fever: Usually above 38.5 °C (101.3 °F), often with sudden onset
  • Chills and a pronounced feeling of illness
  • Burning sensation during urination (dysuria)
  • Frequent urge to urinate with only small amounts of urine
  • Cloudy or foul-smelling urine, possibly with blood (hematuria)
  • Nausea and vomiting
  • General fatigue and weakness

Acute vs. Chronic Pyelonephritis

Acute Pyelonephritis

Acute pyelonephritis presents with sudden and intense symptoms. Affected individuals feel severely unwell. Without treatment, the infection can spread into the bloodstream, causing a life-threatening condition called urosepsis.

Chronic Pyelonephritis

Chronic pyelonephritis often develops gradually with milder symptoms, such as persistent low back pain, fatigue, or occasional low-grade fever. Over time, it can lead to permanent kidney damage and reduced kidney function.

Causes and Risk Factors

Pyelonephritis frequently develops as a complication of an untreated or ascending urinary tract infection (UTI). Known risk factors include:

  • Female sex (shorter urethra increases infection risk)
  • Pregnancy
  • Urinary tract abnormalities or urinary catheters
  • Diabetes mellitus
  • Kidney stones or urinary obstruction
  • Weakened immune system

Diagnosis

Diagnosis involves a physical examination, urinalysis (including urine culture to identify the causative bacteria), blood tests (white blood cell count, CRP), and imaging such as kidney ultrasound. In severe or complicated cases, a CT scan of the abdomen may be required.

Treatment

Pyelonephritis is treated with antibiotics, ideally selected based on the results of a urine culture and sensitivity test. Mild cases can be managed on an outpatient basis with oral antibiotics. Severe cases, especially those involving vomiting or complications, require hospitalization with intravenous antibiotic therapy. Adequate fluid intake and fever-reducing medications are also recommended.

When to See a Doctor

If pyelonephritis is suspected, medical attention should be sought promptly. This is especially urgent in the following situations:

  • High fever above 38.5 °C (101.3 °F)
  • Severe one-sided flank pain
  • Chills or confusion
  • Pregnancy
  • Symptoms that do not improve within 24 hours
