What is Risedronate?
Risedronate (risedronic acid) is a prescription medication belonging to the class of drugs known as bisphosphonates. It is primarily used to treat and prevent osteoporosis, a condition in which bones become weak and prone to fractures. Risedronate is also used to treat Paget's disease of bone, a chronic disorder that disrupts normal bone remodeling.
Indications
Risedronate is indicated for:
- Postmenopausal osteoporosis to reduce the risk of spinal and hip fractures
- Osteoporosis in men at increased risk of fracture
- Glucocorticoid-induced osteoporosis (bone loss caused by long-term corticosteroid use)
- Paget's disease of bone (osteitis deformans)
Mechanism of Action
Risedronate works by selectively inhibiting osteoclasts, the cells responsible for breaking down bone tissue. The active substance binds to the bone surface and blocks a key enzyme in osteoclast metabolism called farnesyl pyrophosphate synthase. This slows down bone resorption while bone formation continues, leading to an overall increase in bone mineral density (BMD) and a significant reduction in fracture risk.
Dosage and Administration
Risedronate is available in several dosing schedules to suit individual needs:
- 5 mg daily
- 35 mg once weekly
- 75 mg on two consecutive days once monthly
- 150 mg once monthly
Tablets must be taken on an empty stomach with a full glass of plain water (at least 200 ml / 6.7 fl oz). Patients should remain upright for at least 30 minutes after taking the tablet and avoid eating, drinking (except water), or taking other medications during this time to maximize absorption and prevent irritation of the esophagus.
Side Effects
Like all medications, risedronate can cause side effects. Common side effects include:
- Abdominal pain, nausea, diarrhea, or constipation
- Heartburn or esophageal irritation (especially if taken incorrectly)
- Headache
- Muscle and joint pain
Rare but serious side effects include:
- Osteonecrosis of the jaw (bone damage in the jaw, particularly associated with dental procedures)
- Atypical femur fractures (unusual thigh bone fractures with very long-term use)
- Severe skin or eye reactions (very rare)
Contraindications
Risedronate should not be used in patients with:
- Severe renal impairment
- Hypocalcemia (low blood calcium levels)
- Pregnancy or breastfeeding
- Known hypersensitivity to the active substance or any excipients
Treatment Context and Supporting Measures
Risedronate therapy is typically combined with adequate intake of calcium and vitamin D, both of which are essential for healthy bone metabolism. The duration of treatment is determined individually by the treating physician and reviewed regularly. Complementary lifestyle measures include weight-bearing physical activity, a calcium-rich diet, smoking cessation, and limiting alcohol consumption.
