The SDAI (Simplified Disease Activity Index) is a validated clinical measurement tool used to assess disease activity in rheumatoid arthritis (RA). It was developed to provide a quick and practical evaluation of disease progression in rheumatology practice. Unlike more complex scores such as the DAS28, the SDAI is straightforward to calculate as it requires no logarithmic formulas.
How is the SDAI Calculated?
The SDAI is composed of five parameters that are simply added together:
- Tender Joint Count (TJC28): Assessment of 28 defined joints for tenderness
- Swollen Joint Count (SJC28): Assessment of the same 28 joints for swelling
- Patient Global Assessment (PtGA): Scale from 0–10 cm (visual analogue scale)
- Evaluator Global Assessment (EGA): Scale from 0–10 cm
- C-reactive Protein (CRP): in mg/dL
The formula is: SDAI = TJC28 + SJC28 + PtGA + EGA + CRP
Interpretation of SDAI Scores
Disease activity is classified into four categories based on the total score:
- Remission: SDAI ≤ 3.3
- Low disease activity: SDAI > 3.3 to ≤ 11
- Moderate disease activity: SDAI > 11 to ≤ 26
- High disease activity: SDAI > 26
Clinical Relevance
The SDAI is used in rheumatology practice for monitoring disease progression and guiding treatment decisions. It allows clinicians to make therapy adjustments based on a combination of objective and subjective parameters. In particular, achieving remission (SDAI ≤ 3.3) is a central goal of modern treatment strategies such as the treat-to-target approach.
The SDAI correlates well with radiological joint damage and functional impairment, making it a reliable tool for evaluating treatment response in rheumatoid arthritis.
