What Does Sympathetic Mean in Medicine?

In medicine, sympathetic refers to the sympathetic nervous system – one of the two main divisions of the autonomic (vegetative) nervous system. The autonomic nervous system controls involuntary bodily functions such as heart rate, breathing, digestion, and metabolism. The sympathetic nervous system acts in opposition to the parasympathetic nervous system.

Function and Mechanism of Action

The sympathetic nervous system becomes active in situations involving stress, danger, or physical exertion. This pattern of response is commonly known as the fight-or-flight response. It prepares the body to react quickly and efficiently to challenges.

Typical Sympathetic Responses in the Body

  • Heart rate and blood pressure increase
  • Airways dilate to allow greater oxygen intake
  • Pupils dilate for improved distance vision
  • Blood flow to muscles increases
  • Digestive activity is reduced
  • Sweating increases
  • Release of adrenaline and noradrenaline from the adrenal glands

Anatomical Basis

The nerve fibers of the sympathetic nervous system originate in the thoracic and lumbar regions of the spinal cord (thoracolumbar division). From there, they travel to ganglia – nerve clusters that act as relay stations, forwarding signals to the target organs. The primary neurotransmitter acting on effector organs is noradrenaline (also called norepinephrine).

Sympathetic and Parasympathetic Balance

A healthy autonomic nervous system requires a well-balanced interplay between the sympathetic and parasympathetic systems. While the sympathetic system drives activity and stress responses, the parasympathetic system promotes rest, recovery, and digestion. This balance is referred to as autonomic equilibrium.

Chronic overstimulation of the sympathetic nervous system – such as from prolonged stress – can lead to health problems, including:

  • Chronically elevated blood pressure (hypertension)
  • Sleep disorders and fatigue
  • Cardiac arrhythmias
  • Gastrointestinal complaints
  • Anxiety disorders and burnout

Clinical Relevance

Disorders of the sympathetic nervous system are relevant across many medical specialties. In cardiology, it plays a key role in managing high blood pressure and heart failure. In neurology, sympathetic dysfunction is studied in conditions such as Horner syndrome and complex regional pain syndrome (CRPS). In internal medicine, sympathetically mediated responses are important in shock and acute stress reactions.

Medications That Affect the Sympathetic Nervous System

Many common medications target the sympathetic nervous system specifically:

  • Beta-blockers (e.g., metoprolol): inhibit sympathetic effects on the heart
  • Alpha-blockers: act on blood vessels to lower blood pressure
  • Sympathomimetics (e.g., adrenaline, ephedrine): mimic the effects of the sympathetic system and are used in shock and allergic reactions
