Things worth knowing about "TBE tick bite"
TBE (tick-borne encephalitis) is a viral disease transmitted by tick bites that can cause inflammation of the brain and meninges. Vaccination is the most effective preventive measure.
What is TBE?
TBE stands for tick-borne encephalitis, a viral infection transmitted through the bite of infected ticks. The causative agent is the TBE virus, a flavivirus predominantly found in specific risk areas of Central and Eastern Europe as well as Asia. In Germany, Bavaria and Baden-Württemberg are among the main TBE risk regions. The name reflects the peak transmission period in spring and early summer, when ticks are most active.
How is TBE transmitted?
The TBE virus is primarily transmitted through the bite of the castor bean tick (Ixodes ricinus). The virus can enter the bloodstream shortly after the tick attaches, meaning that prompt tick removal does not reliably prevent infection. In rare cases, TBE can also be contracted by consuming unpasteurized milk from infected animals.
Symptoms
TBE typically follows a two-phase course:
- First phase (7–14 days after infection): Flu-like symptoms such as fever, headache, fatigue, and muscle aches. For many people, the illness does not progress beyond this stage.
- Second phase (after a symptom-free interval): In approximately 10% of infected individuals, the central nervous system (CNS) becomes involved, which can manifest as meningitis (inflammation of the meninges), encephalitis (inflammation of the brain), or myelitis (inflammation of the spinal cord).
Symptoms of the second phase may include:
- Severe headache
- High fever
- Neck stiffness
- Paralysis
- Disturbances of consciousness
- Speech impairment
Diagnosis
Diagnosis is confirmed through a blood test detecting specific antibodies against the TBE virus (IgM and IgG). If CNS involvement is suspected, a lumbar puncture (cerebrospinal fluid analysis) is performed. Imaging techniques such as MRI may be used to detect inflammation in the brain.
Treatment
There is no specific antiviral treatment for TBE. Management is therefore symptomatic and supportive:
- Bed rest and physical recovery
- Antipyretic and analgesic medications to reduce fever and pain
- In severe cases: inpatient care, potentially in an intensive care unit
- Physiotherapy and neurological rehabilitation for long-term complications
Prevention and Vaccination
The most effective way to prevent TBE is through vaccination. Health authorities recommend vaccination for all individuals living in or travelling to TBE risk areas. The basic immunization schedule consists of three doses, with booster doses recommended every 3 to 5 years. Additional measures to reduce the risk of tick bites include:
- Wearing light-colored, covering clothing in forests and meadows
- Using insect repellents
- Checking the body thoroughly after outdoor activities
- Removing ticks as quickly as possible using tick tweezers or fine-tipped forceps
