What is Tibolone?
Tibolone is a synthetic steroid hormone used in hormone therapy. It belongs to a class of compounds known as selective tissue estrogenic activity regulators (STEARs) and combines estrogenic, progestogenic, and mildly androgenic properties in a single substance. Tibolone is primarily prescribed for postmenopausal women, meaning women whose last menstrual period occurred at least twelve months prior to treatment.
Indications
Tibolone is mainly used for the following medical purposes:
- Treatment of menopausal symptoms such as hot flushes, night sweats, sleep disturbances, mood changes, and vaginal dryness
- Prevention and treatment of osteoporosis in postmenopausal women at increased risk of fractures
- Support for reduced libido and general well-being during the menopause transition
Mechanism of Action
Tibolone itself is an inactive prodrug that is converted in the body into several active metabolites after ingestion. These metabolites bind to different steroid hormone receptors:
- Estrogenic activity: Relieves menopausal symptoms and protects bone density.
- Progestogenic activity: Prevents excessive stimulation of the uterine lining (endometrium).
- Androgenic activity: May positively influence well-being, energy levels, and sexual desire.
Because of this combined activity, tibolone does not need to be paired with a separate progestogen, unlike some other hormone therapies.
Dosage
The standard dose is 2.5 mg of tibolone daily, taken as an oral tablet at the same time each day. Treatment should not be started until at least twelve months after the last menstrual period, as earlier use may lead to irregular bleeding. The appropriate dosage and duration of treatment are determined individually by the prescribing physician.
Side Effects
Like all medications, tibolone can cause side effects. Common side effects include:
- Vaginal bleeding or spotting, particularly at the start of treatment
- Weight gain
- Fluid retention (oedema)
- Breast tenderness or pain
- Headaches or dizziness
- Changes in the vaginal mucosa
Less common but serious risks include an increased risk of stroke (particularly in older women), an increased risk of breast cancer, and effects on liver function.
Contraindications
Tibolone must not be used in:
- Known or suspected breast or uterine cancer
- Unexplained vaginal bleeding
- Pregnancy or breastfeeding
- Severe liver disease
- History of heart attack, stroke, or thrombosis
- Estrogen-dependent tumors
Treatment Context and Notes
Tibolone is often used as an alternative to conventional combined hormone replacement therapy (HRT), as it provides multiple hormonal effects within a single compound. A careful benefit-risk assessment by a physician should be conducted before starting tibolone therapy. Regular check-ups, including mammography and gynecological examinations, are recommended during treatment. Use should be limited to the lowest effective dose for the shortest duration necessary.
