Anatomical Background

The transverse processes are lateral bony projections of the vertebral bodies. They serve as attachment points for muscles and ligaments of the back. Important nerve fibers that transmit pain signals from deep spinal structures run in close proximity to the transverse processes. A targeted injection into this area can effectively inhibit pain transmission.

Indications

The transverse process block is primarily used for the following conditions:

  • Chronic back pain, particularly in the lumbar spine region
  • Muscle tension and myofascial pain syndromes of the back
  • Postoperative pain following spinal surgery
  • Pain management in vertebral fractures

Procedure

The procedure is typically performed on an outpatient basis under sterile conditions, with the patient lying face down. Image guidance – usually fluoroscopy (X-ray) or ultrasound – is used to precisely control needle placement and ensure accurate delivery of the medication. After skin disinfection and local anesthesia, the injection needle is advanced to the transverse process and the medication is injected. The entire procedure usually takes only a few minutes.

The following substances are typically administered:

  • Local anesthetics (e.g., bupivacaine, lidocaine): Temporarily block pain signal transmission
  • Corticosteroids (e.g., triamcinolone, betamethasone): Reduce local inflammation and prolong the duration of pain relief

A transverse process block can provide significant and lasting pain relief in suitable patients. The duration of effect varies depending on the medication used and individual patient response – it can range from a few days to several weeks or months. The block is often integrated into a comprehensive pain management plan that also includes physiotherapy and pharmacological treatment.

The transverse process block is considered a safe procedure when performed by experienced physicians. Possible risks and side effects include:

  • Temporary increase in pain at the injection site
  • Hematoma (bruising) at the injection site
