What is Octenisept?
Octenisept is a water-based antiseptic solution used on skin and mucous membranes. It contains two active ingredients: octenidine dihydrochloride (0.1%) and phenoxyethanol (2%). It is one of the most widely used wound antiseptics in Europe and is available without a prescription at pharmacies.
Mechanism of Action
Octenidine dihydrochloride is a cationic surfactant molecule that binds to the negatively charged cell membranes of microorganisms. This binding destabilizes and destroys the cell membrane, effectively killing the microorganism. Phenoxyethanol acts as a preservative and enhances the antimicrobial effect by further disrupting membrane function.
- Bactericidal: Effective against gram-positive and gram-negative bacteria, including multidrug-resistant strains such as MRSA
- Fungicidal: Effective against fungi such as Candida species
- Virucidal: Effective against enveloped viruses, including herpes viruses and HIV
- No known resistance: No clinically relevant resistance to octenidine has been reported to date
Indications and Uses
Octenisept is used across a variety of medical and nursing settings:
- Disinfection of acute and chronic wounds (e.g., venous leg ulcers, pressure ulcers)
- Antiseptic treatment of burns and abrasions
- Mucous membrane disinfection in the oral and pharyngeal region
- Preparation of skin and mucous membranes before invasive procedures
- Intimate hygiene and mucosal antisepsis in gynaecological care
Dosage and Application
Octenisept is applied undiluted to the affected skin or mucous membrane. The recommended contact time is at least 1 minute for optimal antiseptic efficacy. It can be applied using a sterile swab or directly from the spray bottle. Dilution is not required for mucosal applications.
Important: Octenisept must not be injected into body cavities or introduced under pressure into deep wound pockets, as this can cause severe tissue damage, including compartment syndrome.
Side Effects
Octenisept is generally well tolerated. Possible side effects include:
- Temporary burning or tingling sensation after application
- Local reactions such as redness or mild irritation on sensitive skin
- Rarely: allergic reactions to one of the active ingredients
Severe tissue damage has been documented when Octenisept was injected under pressure into closed tissue structures such as deep wounds or fistulas, potentially leading to compartment syndrome.
Contraindications and Warnings
- Not suitable for use in enclosed body cavities (e.g., abdominal cavity)
- Do not use as ear drops if the eardrum is perforated (risk of ototoxicity)
- Do not inject into deep tissue pockets or fistulas
- Use with caution in newborns and premature infants due to absorption risk
- Pregnancy and breastfeeding: use only after medical consultation
