What Is an Oral Vaccine?
An oral vaccine is a type of vaccine that is swallowed or administered as drops into the mouth, rather than injected into a muscle or under the skin. This method of immunization is particularly popular in children because it is painless and easy to administer. Well-known examples include the oral polio vaccine (OPV) and the rotavirus oral vaccine.
Mechanism of Action
After ingestion, the vaccine travels through the gastrointestinal tract, where it is taken up by the gut-associated lymphoid tissue (GALT), also referred to as the mucosa-associated lymphoid tissue (MALT). This triggers a specific immune response that not only protects locally in the intestines but can also build systemic immunity throughout the body. Furthermore, oral vaccines stimulate the production of secretory immunoglobulin A (sIgA) in the mucous membranes, which is particularly effective against pathogens that enter the body through the digestive tract.
Areas of Application
Oral vaccines are used to protect against several diseases, including:
- Poliomyelitis (polio): The oral polio vaccine (OPV, Sabin vaccine) was the global standard for decades and remains in use in many countries.
- Rotavirus infection: An oral vaccine for infants to prevent severe diarrhea and vomiting caused by rotavirus.
- Typhoid fever: An oral live typhoid vaccine (Ty21a) is available for travelers to endemic regions.
- Cholera: An oral cholera vaccine is recommended for travelers to high-risk areas.
Advantages of Oral Vaccines
Compared to traditional injections, oral vaccines offer several benefits:
- Painless administration – ideal for infants and young children
- No needles – reduces fear and discomfort
- Easy and fast to administer, even during mass vaccination campaigns
- Directly stimulates mucosal immunity in the gastrointestinal tract
- Can contribute to herd immunity, as vaccinated individuals are less likely to shed the pathogen
Side Effects and Risks
Oral vaccines are generally considered safe and well tolerated. Possible side effects are usually mild and may include:
- Temporary gastrointestinal discomfort such as diarrhea or bloating
- Mild nausea
A very rare risk associated with the oral polio live vaccine (OPV) is vaccine-associated paralytic poliomyelitis (VAPP), in which the attenuated virus can, in extremely rare cases, revert to a more infectious form. For this reason, many high-income countries, including Germany, now exclusively use the inactivated polio vaccine (IPV) given by injection. However, OPV remains a critical tool for global polio eradication efforts in developing countries.
Oral Vaccines in Vaccination Schedules
In Germany, the Standing Committee on Vaccination (STIKO) at the Robert Koch Institute currently recommends the rotavirus oral vaccine for infants from the age of six weeks onwards. This vaccine must be completed within a specific age window. The oral polio vaccine is no longer part of the standard German vaccination schedule; instead, polio vaccination is administered by injection as part of a combination vaccine.
