What is Oxymetazoline?

Oxymetazoline is an alpha-adrenergic sympathomimetic agent primarily used as a topical nasal decongestant. It belongs to the class of vasoconstrictive substances and is available over the counter under various brand names. The drug is applied locally and acts directly on the mucosal blood vessels of the nose, and in certain formulations, the eyes.

Indications

Oxymetazoline is used for the following conditions:

• Nasal congestion caused by common colds, flu-like infections, and acute sinusitis
• Allergic rhinitis (hay fever) for short-term symptom relief
• Middle ear infections to improve ventilation via the Eustachian tube
• Conjunctivitis (eye redness) in specialized ophthalmic formulations
• Postoperative nasal care in selected ENT surgical situations

Mechanism of Action

Oxymetazoline acts as a selective alpha-2 adrenoceptor agonist and, at higher concentrations, also as an alpha-1 adrenoceptor agonist. Activation of these receptors in the walls of the nasal mucosal blood vessels causes vasoconstriction -- a narrowing of the small blood vessels. This reduces blood flow to the swollen mucosa, decreases mucosal edema, and typically restores free nasal breathing within minutes. The effect generally lasts 8 to 12 hours.

Dosage and Usage

Oxymetazoline is available in different concentrations:

• 0.05% (0.5 mg/ml): Standard dosage for adults and children aged 6 and older
• 0.025% (0.25 mg/ml): For children aged 2 to 6 years (only on medical advice)

The recommended duration of use is no more than 3 to 7 days, as prolonged use can lead to a rebound effect known as rhinitis medicamentosa. Typically, 1 to 2 sprays per nostril, 2 to 3 times daily, are recommended. Oxymetazoline should not be used in children under 2 years of age.

Side Effects

When used as directed, oxymetazoline is generally well tolerated. Possible side effects include:

• Burning or dryness in the nasal passages
• Sneezing immediately after application
• With overuse or prolonged application: rebound congestion (increased nasal swelling once the drug wears off)
• Rarely: rapid heartbeat, elevated blood pressure, insomnia (especially with systemic absorption)
• Rhinitis medicamentosa: a form of drug-induced nasal congestion from continuous use beyond 7 days

Contraindications and Precautions

Oxymetazoline should not be used or should only be used under medical supervision in:

• Known hypersensitivity to the active ingredient
• Narrow-angle glaucoma (increased intraocular pressure)
• Severe heart disease or uncontrolled high blood pressure
• Concurrent use of MAO inhibitors (risk of serious interaction)
• Pregnancy and breastfeeding (consult a physician before use)
• Children under 2 years of age (contraindicated)

Clinical Significance

Oxymetazoline is one of the most widely used nasal decongestants worldwide. It provides rapid and effective relief from nasal congestion. However, short-term use is essential to avoid medication-induced rhinitis. For chronic nasal congestion, underlying causes should be investigated by a healthcare professional, and longer-acting treatments such as intranasal corticosteroids should be considered.
