Xarelto (Rivaroxaban) – Uses, Dosage & Side Effects

What is Xarelto?

Xarelto is a prescription anticoagulant medication containing the active ingredient rivaroxaban. It belongs to the class of direct oral anticoagulants (DOACs) and is used to prevent and treat dangerous blood clots. Xarelto is developed and marketed by the pharmaceutical company Bayer.

Indications

Xarelto is approved for several medical conditions, including:

• Prevention of stroke and systemic embolism in adults with non-valvular atrial fibrillation
• Treatment and prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE)
• Prevention of venous thromboembolism following elective hip or knee replacement surgery
• Prevention of atherothrombotic events in adults after acute coronary syndrome (in combination with antiplatelet therapy)
• Prevention of atherothrombotic events in adults with coronary artery disease or peripheral artery disease

Mechanism of Action

Rivaroxaban, the active ingredient in Xarelto, is a direct Factor Xa inhibitor. Factor Xa plays a central role in the blood coagulation cascade. By specifically blocking this factor, rivaroxaban prevents the conversion of prothrombin into thrombin, which in turn stops the formation of fibrin and blood clots. Unlike traditional anticoagulants such as warfarin, rivaroxaban acts directly without requiring metabolic activation.

Dosage

The recommended dose of Xarelto depends on the indication:

• Atrial fibrillation (stroke prevention): 20 mg once daily (15 mg once daily in patients with moderate renal impairment), taken with the evening meal
• Treatment of DVT and PE: 15 mg twice daily for the first 3 weeks, followed by 20 mg once daily
• Post-operative thromboprophylaxis (hip/knee replacement): 10 mg once daily
• Coronary artery disease / peripheral artery disease: 2.5 mg twice daily (in combination with aspirin)

Xarelto is taken orally as a tablet. The 15 mg and 20 mg tablets should be taken with food to ensure adequate absorption.

Side Effects

Like all anticoagulants, Xarelto can cause side effects. The most clinically significant risk is increased bleeding.

Common Side Effects

• Bleeding (e.g., nosebleeds, bleeding gums, bruising)
• Nausea and vomiting
• Abdominal pain
• Elevated liver enzyme levels

Serious Side Effects

• Internal bleeding (e.g., gastrointestinal, intracranial)
• Severe allergic reactions
• Liver disorders

Patients should seek immediate medical attention if they experience unusual or prolonged bleeding, blood in their stool or urine, severe headaches, or visual disturbances.

Interactions and Contraindications

Xarelto should not be used in patients with:

• Active clinically significant bleeding
• Liver disease associated with coagulopathy and clinically relevant bleeding risk
• Pregnancy or breastfeeding
• Concurrent use of other anticoagulants (except in specific, clinically defined situations)

Clinically relevant drug interactions occur with azole antifungals (e.g., ketoconazole), HIV protease inhibitors, certain antibiotics, and other blood-thinning agents. Co-administration of St. John's Wort (Hypericum perforatum) may reduce the effectiveness of Xarelto.

Important Notes

Patients taking Xarelto should:

• Always inform their doctor, dentist, and other healthcare providers about their anticoagulant therapy
• Never stop taking the medication abruptly without consulting their doctor
• Attend regular follow-up appointments
• Carry an anticoagulant patient card at all times

Unlike vitamin K antagonists (e.g., warfarin), Xarelto does not require routine INR monitoring. In the event of severe or life-threatening bleeding, andexanet alfa is available as a specific reversal agent.
