What is Xylometazoline?
Xylometazoline is a locally acting decongestant belonging to the group of alpha-sympathomimetics. It is primarily used in nasal sprays and nasal drops to relieve nasal congestion caused by colds, allergies, or sinus infections. Xylometazoline is widely available as an over-the-counter medication in many countries and is one of the most commonly used nasal decongestants worldwide.
Mechanism of Action
Xylometazoline acts as an alpha-1 and alpha-2 adrenergic receptor agonist. By binding to these receptors on the smooth muscle cells of blood vessels in the nasal mucosa, it causes vasoconstriction -- a narrowing of the small arteries and veins. This reduces blood flow to the nasal lining, decreases swelling, and opens up the nasal passages. The onset of action is typically within a few minutes, and effects last for 8 to 12 hours depending on the formulation.
Indications
- Acute and chronic nasal congestion due to the common cold
- Allergic rhinitis (hay fever)
- Sinusitis (inflammation of the paranasal sinuses)
- Facilitating nasal breathing before diagnostic or surgical procedures
- Otitis media (to relieve pressure via the Eustachian tube)
Dosage and Administration
Xylometazoline is available in different concentrations:
- 0.05%: for children aged 2 years and older (in special formulations, also for infants from 3 months under medical supervision)
- 0.1%: for adults and adolescents aged 12 years and older
The recommended duration of use is generally no more than 5 to 7 days. Prolonged use without medical advice is discouraged due to the risk of rebound congestion and dependency.
Side Effects
When used as directed, xylometazoline is generally well tolerated. Possible side effects include:
- Burning or dryness in the nose
- Sneezing
- Headache
- With overdose or accidental ingestion: increased blood pressure, rapid heartbeat, drowsiness (especially in children)
- With prolonged use: rhinitis medicamentosa (rebound congestion), also known as Privin dependency
Contraindications and Interactions
Xylometazoline should not be used in patients with:
- Atrophic rhinitis (rhinitis sicca)
- Narrow-angle glaucoma
- Concurrent use of MAO inhibitors or tricyclic antidepressants
- Known hypersensitivity to the active substance
Caution is also advised during pregnancy and breastfeeding, and in patients with cardiovascular disease, hypertension, hyperthyroidism, or diabetes. These patients should consult a healthcare professional before use.
