What Is a Differential Blood Count?

A differential blood count (also called a leukocyte differential or diff count) is an advanced blood test that is typically part of a complete blood count (CBC). It measures and reports both the percentage and the absolute number of each type of white blood cell (leukocyte) in a blood sample. This test is a key diagnostic tool that helps physicians detect and classify infections, immune system disorders, and hematological conditions.

Which Cells Are Analyzed?

The differential blood count evaluates five main types of white blood cells:

  • Neutrophils: The most abundant white blood cells; they are the first line of defense against bacterial infections.
  • Lymphocytes: Key cells of the adaptive immune system, including B cells, T cells, and natural killer cells.
  • Monocytes: Large phagocytic cells that engulf and destroy pathogens and cellular debris.
  • Eosinophils: Involved in allergic reactions and the defense against parasitic infections.
  • Basophils: The rarest white blood cells; they play a role in allergic and inflammatory responses.

When Is a Differential Blood Count Ordered?

A differential blood count is ordered in a variety of clinical situations, including:

  • Suspected bacterial or viral infections
  • Monitoring of immune system disorders (e.g., autoimmune diseases)
  • Evaluation of blood disorders such as leukemia or anemia
  • Follow-up during chemotherapy or immunosuppressive therapy
  • Allergological workup (elevated eosinophils)
  • Routine health checkups and preventive care

How Is the Test Performed?

A venous blood sample is drawn and analyzed, usually by automated hematology analyzers. When results are abnormal or require verification, a manual differential is performed by a trained laboratory professional or hematologist, who examines and counts the cells under a microscope using a stained blood smear.

Normal Values and Interpretation

Reference ranges may vary slightly between laboratories. Typical values for adults are:

  • Neutrophils: 50–70% (absolute: 1.8–7.5 × 10⁹/l)
  • Lymphocytes: 20–40% (absolute: 1.0–4.0 × 10⁹/l)
  • Monocytes: 2–8% (absolute: 0.2–1.0 × 10⁹/l)
  • Eosinophils: 1–4% (absolute: 0.04–0.4 × 10⁹/l)
  • Basophils: 0–1% (absolute: 0.01–0.1 × 10⁹/l)

Deviations from these ranges may indicate various conditions. Elevated neutrophils (neutrophilia) often suggest a bacterial infection, while elevated lymphocytes (lymphocytosis) are typical of viral infections. Elevated eosinophils (eosinophilia) may point to allergies or parasitic infestation.

Common Abnormalities and Their Significance

Elevated Values

  • Neutrophilia: Bacterial infections, inflammation, physical stress, corticosteroid therapy
  • Lymphocytosis: Viral infections (e.g., mononucleosis, COVID-19), chronic lymphocytic leukemia
  • Eosinophilia: Allergies, asthma, parasitic infections, certain medications
  • Monocytosis: Chronic infections (e.g., tuberculosis), inflammatory bowel disease

Decreased Values

  • Neutropenia: Increased risk of infection; may be caused by chemotherapy, autoimmune conditions, or bone marrow disorders
  • Lymphopenia: HIV infection, immunosuppressive therapy, severe systemic infections
