What is a Ketolide Antibiotic?
Ketolide antibiotics are a modern subclass of macrolide antibiotics, developed specifically to combat bacterial infections resistant to older macrolides such as erythromycin. The most well-known member of this class is telithromycin (brand name: Ketek). Ketolides feature a modified chemical structure that provides stronger binding to bacterial ribosomes and a broader spectrum of activity.
Mechanism of Action
Ketolides act as protein synthesis inhibitors. They bind to the 23S rRNA of the bacterial 50S ribosomal subunit, thereby blocking protein synthesis within the bacterial cell. Compared to classic macrolides, ketolides bind simultaneously at two sites on the ribosome, significantly increasing binding affinity and enabling activity even against macrolide-resistant pathogens.
Indications
Ketolide antibiotics are primarily used to treat community-acquired respiratory tract infections, including:
- Community-acquired pneumonia
- Acute exacerbations of chronic bronchitis
- Acute bacterial sinusitis
- Tonsillopharyngitis (tonsil and throat infections)
They are particularly effective against typical and atypical pathogens such as Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila.
Dosage
Telithromycin, the most prominent ketolide, is typically administered orally at a dose of 800 mg once daily. Treatment duration ranges from 5 to 10 days depending on the indication. It can be taken with or without food. Dose adjustments may be necessary in patients with impaired kidney function.
Side Effects
Like all antibiotics, ketolides may cause side effects. The most common include:
- Nausea, vomiting, and diarrhea
- Headache and dizziness
- Visual disturbances (transient accommodation disorders)
- Taste disturbances
Serious, though rare, side effects include hepatotoxicity (liver damage) and QT interval prolongation on the ECG, which can lead to cardiac arrhythmias. Due to reports of severe liver injury, the approved indications for telithromycin have been restricted in many countries.
Contraindications and Interactions
Ketolides should not be used in patients with:
- Known hypersensitivity to macrolide antibiotics or ketolides
- Severe hepatic impairment
- Myasthenia gravis (a neuromuscular disorder, as ketolides can worsen this condition)
- Concomitant use of medications that prolong the QT interval
Important drug interactions exist with statins (cholesterol-lowering drugs) such as simvastatin and atorvastatin, certain immunosuppressants, and CYP3A4 inhibitors, as ketolides inhibit this liver enzyme and can increase plasma levels of co-administered drugs.
Clinical Relevance and Current Use
Since the introduction of telithromycin, safety concerns -- particularly regarding hepatotoxicity -- have led to restrictions or withdrawal of approvals in various countries. In the European Union, telithromycin remains approved but only for specific indications and with enhanced safety warnings. Research into new ketolide compounds such as solithromycin aims to preserve the therapeutic benefits of the class while improving the safety profile.
