What is Rhinotillexomania?
Rhinotillexomania refers to the compulsive, uncontrollable urge to pick one nose. The term derives from the Greek words rhino (nose), tillexis (scratching), and mania (compulsion). Unlike occasional nose-picking, which is common for most people, rhinotillexomania is a repetitive, compulsive behavior that can cause significant distress or physical harm. It is classified within the spectrum of obsessive-compulsive spectrum disorders.
Causes
The exact causes of rhinotillexomania are not fully understood. The following factors are discussed:
- Psychological factors: Stress, anxiety, and emotional tension can trigger or worsen compulsive behavior.
- Obsessive-compulsive disorders: Rhinotillexomania frequently co-occurs with OCD, body dysmorphic disorder, or other repetitive behaviors.
- Neurological factors: Changes in brain regions responsible for impulse control may play a role.
- Habit formation: Initially harmless habits can over time develop into compulsive patterns.
- Physical triggers: Dry nasal mucosa, allergies, or colds can intensify the urge.
Symptoms
Typical features of rhinotillexomania include:
- Repetitive, difficult-to-control insertion of fingers into the nose
- Spending more than one hour per day on this behavior
- Significant distress or social impairment
- Nosebleeds due to injuries to the nasal mucosa
- Inflammation or infections inside the nose
- In extreme cases: perforation of the nasal septum
- Feelings of shame and guilt following the behavior
Diagnosis
Diagnosis is primarily clinical, as no specific laboratory tests exist. A physician or psychologist evaluates:
- The frequency and intensity of the behavior
- The resulting level of distress
- Co-occurring mental health conditions (e.g., OCD, ADHD)
- Physical damage to the nasal mucosa or nasal septum
It is important to differentiate rhinotillexomania from other repetitive behaviors such as dermatillomania (skin picking) or trichotillomania (hair pulling), with which it frequently co-occurs.
Treatment
Treatment depends on the severity of the condition and any accompanying disorders:
Psychotherapy
Cognitive Behavioral Therapy (CBT) is the most effective treatment approach. Habit Reversal Training (HRT) has proven particularly effective, teaching individuals to replace the unwanted behavior with alternative actions. Acceptance and Commitment Therapy (ACT) may also be beneficial.
Medication
In severe cases or when obsessive-compulsive disorders are present, selective serotonin reuptake inhibitors (SSRIs) may be prescribed, as they are effective in treating other obsessive-compulsive spectrum disorders as well.
Supportive Measures
- Moisturizing the nasal mucosa with saline spray to reduce physical triggers
- Stress management and relaxation techniques
- Support groups and psychoeducation about the condition
