What Are Xanthomas?
Xanthomas are yellowish deposits of lipids (fats) that accumulate in the skin or deeper tissues. They typically appear as a visible sign of an underlying lipid metabolism disorder, such as elevated cholesterol or triglyceride levels in the blood. Xanthomas can develop in various locations, including the eyelids (xanthelasmas), tendons, elbows, and hands.
Causes of Xanthomas
The most common causes of xanthoma formation include:
- Primary hyperlipidemias: Genetically determined lipid disorders such as familial hypercholesterolemia
- Secondary hyperlipidemias: Elevated blood lipid levels resulting from conditions such as diabetes mellitus, hypothyroidism, or liver disease
- Dietary factors: Excessive intake of saturated fats and dietary cholesterol
Goals of Xanthoma Therapy
Xanthoma therapy pursues two main goals: first, treating the underlying cause by lowering elevated blood lipid levels, and second, directly removing or reducing the visible fatty deposits in the skin.
Treatment Approaches
1. Treatment of the Underlying Condition
Since xanthomas are usually a symptom of a lipid metabolism disorder, treating the root cause is the primary focus:
- Dietary modification: Reduction of saturated fatty acids, trans fats, and cholesterol-rich foods; increased intake of fiber and unsaturated fatty acids
- Pharmacological therapy: Use of lipid-lowering agents such as statins, fibrates, ezetimibe, or – in cases of severe familial hypercholesterolemia – PCSK9 inhibitors
- Management of comorbidities: Optimal control of diabetes mellitus, thyroid disorders, or other underlying conditions
2. Direct Treatment of Xanthomas
Once blood lipid levels are successfully reduced, xanthomas may partially regress on their own. If they persist, the following procedures are available for direct removal:
- Surgical excision: Operative removal of fatty deposits, particularly for larger or troublesome xanthomas
- Laser treatment: Use of CO2 laser or Erbium-YAG laser for precise ablation, especially for xanthelasmas on the eyelids
- Cryotherapy: Freezing of the affected skin areas to destroy the fatty deposits
- Chemical methods: Topical application of trichloroacetic acid (TCA) to dissolve xanthelasmas
- Electrocautery: Obliteration of deposits using electrical current
Prognosis and Outlook
The prognosis of xanthoma therapy largely depends on the successful management of the underlying cause. With consistent reduction of blood lipid levels, xanthomas can disappear completely or partially. However, after local removal procedures, there is a risk of recurrence if the lipid disorder is not kept under long-term control. Ongoing medical follow-up is therefore strongly recommended.
