What is the Venous Pressure Test?
The venous pressure test (also called venous pressure measurement) is a medical diagnostic procedure used to assess the function of the venous system. It measures the pressure inside the veins to evaluate blood circulation, venous valve function, and to identify conditions such as chronic venous insufficiency (CVI), varicose veins, or deep vein thrombosis (DVT).
How Does the Venous Pressure Test Work?
In the direct, invasive form of the test, a thin needle is inserted into a superficial vein, typically on the dorsum of the foot. A pressure transducer measures venous pressure in millimeters of mercury (mmHg) both at rest and during physical activity (e.g., toe-raise exercises).
Non-invasive methods are also commonly used, including:
- Light Reflection Rheography (LRR): An optical technique that measures changes in skin blood flow to indirectly assess venous pressure.
- Photoplethysmography (PPG): Similar to LRR, this method optically measures venous filling and emptying time in the leg veins.
- Doppler Ultrasound Venous Pressure Assessment: An imaging method that evaluates blood flow and venous pressure without any invasive procedure.
When is the Venous Pressure Test Used?
The venous pressure test is indicated in the following situations:
- Heavy, swollen, or painful legs
- Visible varicose veins or spider veins
- Suspected chronic venous insufficiency
- Evaluation for deep vein thrombosis
- Follow-up after venous procedures (e.g., sclerotherapy or stripping)
- Pre-operative assessment before venous surgery
What Do the Results Mean?
Under normal conditions, venous pressure drops significantly during physical activity (due to the muscle pump effect) and rises again slowly once activity stops. In cases of venous valve incompetence or venous reflux, the pressure drop is minimal, or the pressure rises again too quickly -- both indicating impaired venous valve function.
- Normal venous pressure at rest (lying down): approximately 10–15 mmHg
- Normal venous pressure while standing: up to 80–100 mmHg (hydrostatic pressure)
- Pressure drop during exercise: Healthy veins show a significant pressure reduction of at least 50% during activity.
Diagnosis and Clinical Significance
The venous pressure test provides important information for diagnosing and planning treatment of venous conditions. It allows an objective assessment of the severity of venous insufficiency and helps determine the most suitable treatment -- ranging from conservative measures such as compression therapy to interventional or surgical procedures.
Risks and Side Effects
Non-invasive methods (LRR, PPG) are completely risk-free and painless. The invasive needle-based measurement may involve minor side effects:
- Brief pain or pressure at the puncture site
- Minor bleeding tendency
- Rarely: local inflammatory reaction
Overall, the venous pressure test is considered a safe and well-tolerated diagnostic procedure.
