Night sweats refer to repeated episodes of heavy sweating during sleep that soak clothing or bedding. Causes range from hormonal changes to serious medical conditions.
What Are Night Sweats?
Night sweats (medically known as hyperhidrosis nocturna) are recurring episodes of excessive sweating during sleep that occur independently of an overly warm sleeping environment. Affected individuals often wake up with drenched clothing or bedding. While occasional nighttime sweating is normal, persistent night sweats can indicate an underlying medical condition and should be evaluated by a healthcare professional.
Causes
The causes of night sweats are diverse. They range from harmless, physiological triggers to conditions requiring medical treatment.
Common Harmless Causes
- Warm sleeping environment: Excessive room temperature or inappropriate bedding
- Hormonal fluctuations: Especially during menopause in women, but also in men due to declining testosterone levels
- Pregnancy: Hormonal changes can cause nighttime sweating
- Stress and anxiety disorders: Psychological burden activates the nervous system and can trigger sweating
- Alcohol and caffeine consumption: Stimulants can affect the body's temperature regulation
Medical Causes
- Infectious diseases: Such as tuberculosis, HIV, or other bacterial and viral infections
- Hormonal disorders: Overactive thyroid (hyperthyroidism), pheochromocytoma, or carcinoid syndrome
- Malignant conditions: Certain cancers, such as lymphomas (e.g., Hodgkin lymphoma), can cause night sweats as a so-called B symptom
- Autoimmune diseases: Rheumatoid arthritis or other inflammatory conditions
- Medications: Antidepressants, antipyretics, certain blood pressure medications, or hormones can cause night sweats as a side effect
- Neurological conditions: Autonomic neuropathies or nervous system disorders
Symptoms and Associated Complaints
Night sweats typically present as intense, diffuse sweating that can affect the entire body. Depending on the underlying cause, additional symptoms may occur:
- Unintentional weight loss
- Fever or chills
- Fatigue and persistent tiredness
- Swollen lymph nodes
- Sleep disturbances and poor sleep quality
- Heart palpitations or racing heartbeat
If night sweats are accompanied by unintentional weight loss, fever, or swollen lymph nodes, medical attention should be sought promptly, as this combination may indicate a more serious underlying condition.
Diagnosis
To evaluate night sweats, a doctor will first take a detailed medical history. Key questions include: How often do the sweats occur? Are there accompanying symptoms? What medications are being taken?
The following diagnostic measures may then be initiated:
- Blood tests: Complete blood count, thyroid levels (TSH, fT3, fT4), inflammatory markers (CRP, ESR), and hormone levels
- Urine analysis: To rule out infections or hormone-producing tumors
- Imaging procedures: Ultrasound, X-ray, or CT scan to assess internal organs and lymph nodes
- Specialized tests: Depending on suspicion, e.g., tuberculin test, HIV test, or further hormone analyses
Treatment
Treatment of night sweats is directed at the underlying cause:
General Measures
- Optimizing the sleep environment (cool room temperature, breathable bedding)
- Avoiding alcohol, caffeine, and spicy foods before bedtime
- Stress reduction through relaxation techniques, yoga, or progressive muscle relaxation
- Regular physical activity
Medical Treatment
- Hormone therapy: For menopausal symptoms, hormone replacement therapy may be considered in consultation with a physician
- Thyroid treatment: In cases of hyperthyroidism, appropriate medications (e.g., antithyroid drugs) are used
- Antibiotics or antiviral agents: For infectious diseases
- Oncological therapy: If night sweats are cancer-related, the underlying malignancy is treated
- Medication adjustment: If a medication is identified as the trigger, switching or adjusting the dosage may help
When to See a Doctor
A medical consultation is recommended if night sweats:
- Occur regularly and without an identifiable reason
- Significantly disrupt sleep
- Are accompanied by fever, weight loss, or swollen lymph nodes
- Have appeared suddenly and for the first time
- Persist despite improving the sleep environment
